清金化痰汤加减治疗晚期肺癌合并肺部感染的临床效果

摘　　要：目的探讨清金化痰汤加减治疗晚期肺癌合并肺部感染的临床效果。方法选取2023年1月—2024年1月菏泽市中医医院收治的晚期肺癌合并肺部感染患者80例,按照治疗方法分为观察组和对照组,各40例。对照组给予肺癌晚期标准化疗方案+注射用头孢哌酮钠舒巴坦钠静脉滴注治疗,观察组在对照组治疗基础上给予清金化痰汤加减治疗。比较两组治疗后临床疗效、临床症状评分和血清炎症因子水平以及治疗期间不良反应发生情况。结果治疗7 d后,观察组总有效率高于对照组;发热、咳嗽、咳痰和肺部体征等临床症状评分和血清C反应蛋白、降钙素原、白细胞介素-6水平低于对照组(P<0.05)。治疗期间,观察组白细胞减少和血小板减少发生率低于对照组(P<0.05);两组胃肠道反应、肝功能损害和骨髓抑制发生率比较,差异无统计学意义(P>0.05)。结论清金化痰汤加减治疗晚期肺癌合并肺部感染的临床效果显著,能有效改善患者临床症状,降低血清炎症因子水平,且安全性较好。Objective To explore the clinical effect of Qingjin Huatan Decoction in the treatment of advanced lung cancer with pulmonary infection.Methods A total of 80 patients with advanced lung cancer complicated with pulmonary infection admitted to Heze Hospital of Traditional Chinese Medicine from January 2023 to January 2024 were selected and divided into observation group and control group according to treatment method,with 40 cases in each group.The control group was treated with standard chemotherapy for advanced lung cancer+intravenous infusion of Cefoperazone Sodium and Sulbactam Sodium for Injection.The observation group was treated with Qingjin Huatan Decoction on the basis of the control group.The clinical efficacy,clinical symptom score,serum inflammatory factor level and adverse reactions during treatment were compared between the two groups.Results After 7 days of treatment,the total effective rate of the observation group was higher than that of the control group.The scores of clinical symptoms such as fever,cough,sputum and pulmonary signs and the levels of serum C reactive protein,procalcitonin and interleukin-6 were lower than those in the control group(P<0.05).During the treatment,the incidence of leukopenia and thrombocytopenia in the observation group was lower than that in the control group,(P<0.05).There was no significant difference in the incidence of gastrointestinal reaction,liver function damage and bone marrow suppression between the two groups(P>0.05).Conclusions Qingjin Huatan Decoction has a significant clinical effect in the treatment of advanced lung cancer complicated with pulmonary infection.It can effectively improve the clinical symptoms of patients,reduce the level of serum inflammatory factors,and has good safety.
