摘　　要：目的探究糖脂代谢、血清高迁移率族蛋白1(HMGB1)与胰腺炎严重程度的相关性及对患者预后的预测价值。方法回顾性选择2022年1月至2024年1月于西安交通大学附属红会医院接受治疗的94例胰腺炎患者作为研究对象,纳入胰腺炎组。同期选择94名体检健康者作为对照组。将94例胰腺炎患者按严重程度分为重度胰腺炎组24例、中度胰腺炎组35例、轻度胰腺炎组35例;按出院时是否死亡分为预后不良组19例和预后良好组75例。比较不同组血清糖脂代谢指标[空腹血糖、甘油三酯、总胆固醇、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)]及血清HMGB1水平,通过多因素Logistic回归分析探讨影响胰腺炎患者不良预后的因素,并采用受试者操作特征(ROC)曲线分析血清糖脂代谢指标及血清HMGB1水平对胰腺炎患者预后的预测价值。结果胰腺炎组患者空腹血糖、甘油三酯、总胆固醇、HDL、HMGB1水平分别为(7.38±1.24)mmol/L、(3.02±0.76)mmol/L、(4.76±0.81)mmol/L、(1.92±0.37)mmol/L、(2.01±0.43)μg/L,均显著高于对照组[(5.55±0.89)mmol/L、(2.29±0.45)mmol/L、(3.45±0.77)mmol/L、(1.49±0.21)mmol/L、(0.83±0.12)μg/L],LDL水平为(1.19±0.21)mmol/L,显著低于对照组[(1.45±0.25)mmol/L],差异均有统计学意义(P<0.05)。重度胰腺炎组患者空腹血糖、甘油三酯、总胆固醇、HDL、HMGB1水平均显著高于中度胰腺炎组、轻度胰腺炎组,LDL水平显著低于中度胰腺炎组、轻度胰腺炎组,且中度胰腺炎组患者空腹血糖、甘油三酯、总胆固醇、HDL、HMGB1水平均显著高于轻度胰腺炎组,LDL水平显著低于轻度胰腺炎组,差异均有统计学意义(P<0.05)。预后不良组患者空腹血糖、甘油三酯、总胆固醇、HDL、HMGB1水平均显著高于预后良好组,LDL水平低于预后良好组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果表明,血清空腹血糖、甘油三酯、总胆固醇、HDL、LDL、HMGB1水Objective To investigate the correlation between glycolipid metabolism,serum high mobility group box 1(HMGB1)and the severity of pancreatitis,as well as their predictive value for prognosis.Methods A total of 94 patients with pancreatitis who were treated in Xi'an Jiaotong University Affiliated Honghui Hospital from January 2022 to January 2024 were retrospectively selected as the study subjects and included in the pancreatitis group.At the same time,94 healthy subjects were selected as the control group.According to the severity,94 patients with pancreatitis were divided into 24 cases in the severe pancreatitis group,35 cases in the moderate pancreatitis group and 35 cases in the mild pancreatitis group.According to whether they died at discharge,they were divided into 19 cases in the poor prognosis group and 75 cases in the good prognosis group.The serum glycolipid metabolism indicators[fasting blood glucose,triglyceride,total cholesterol,high density lipoprotein(HDL)and low density lipoprotein(LDL)]and serum HMGB1 levels were compared among different groups.The factors affecting the poor prognosis of patients with pancreatitis were explored by multivariate Logistic regression analysis.The predictive value of serum glucose and lipid metabolism indexes and serum HMGB1 level on the prognosis of patients with pancreatitis was analyzed by receiver operating characteristic(ROC)curve.Results The levels of fasting blood glucose,triglyceride,total cholesterol,HDL and HMGB1 in pancreatitis group were(7.38±1.24)mmol/L,(3.02±0.76)mmol/L,(4.76±0.81)mmol/L,(1.92±0.37)mmol/L and(2.01±0.43)μg/L,respectively,which were significantly higher than those in the control group[(5.55±0.89)mmol/L,(2.29±0.45)mmol/L,(3.45±0.77)mmol/L,(1.49±0.21)mmol/L,(0.83±0.12)μg/L],LDL level was(1.19±0.21)mmol/L,which was significantly lower than that in the control group[(1.45±0.25)mmol/L],the differences were statistically significant(P<0.05).The levels of fasting blood glucose,triglyceride,total cholesterol,HDL and HMGB1 in the sever
