摘　　要：目的分析结直肠癌组织核仁纺锤体相关蛋白质1(NUSAP1)、分泌型蛋白1(DKK1)阳性表达与病理特征的关系,并探讨其对患者预后的影响。方法选取2020年1月至2021年6月漯河市中心医院收治的95例结直肠癌患者作为研究对象。免疫组化法分析结直肠癌组织及癌旁组织中NUSAP1、DKK1阳性表达,采用受试者工作特征(ROC)曲线及曲线下面积(AUC)评估NUSAP1、DKK1联合检测对结直肠癌的诊断价值。比较结直肠癌不同病理特征患者的NUSAP1、DKK1阳性表达水平,采用Kaplan-Meier生存曲线分析结直肠癌患者NUSAP1、DKK1表达预后的生存情况,COX回归分析结直肠癌患者预后的影响因素。结果结直肠癌患者的癌组织NUSAP1阳性表达为78.95%,明显高于癌旁组织的13.68%,DKK1阳性表达为54.74%,明显低于癌旁组织的72.63%,差异均有统计学意义(P<0.05);ROC曲线分析结果显示,NUSAP1、DKK1单独及联合诊断结直肠癌的AUC为0.826(95%CI:0.765~0.877)、0.589(95%CI:0.516~0.660)、0.900(95%CI:0.848~0.939),联合诊断的AUC较高,最佳敏感性、特异性分别为89.47%、90.53%;不同TNM分期、肿瘤直径、淋巴结转移、远处转移患者NUSAP1、DKK1阳性表达比较差异均有统计学意义(P<0.05);Kaplan-Meier曲线分析结果显示,NUSAP1阳性表达患者3年生存率为60.29%,明显低于阴性表达者的89.27%,DKK1阳性表达患者的3年生存率为77.08%,明显高于阴性表达者的53.85%,差异均有统计学意义(P<0.05);COX回归多因素结果显示,TNM分期、NUSAP1阳性、淋巴结转移、DKK1阳性、远处转移均是结肠癌预后的影响因素(P<0.05)。结论结直肠癌组织患者NUSAP1、DKK1阳性表达与病理特征及预后密切相关,且NUSAP1、DKK1联合可用于辅助诊断结直肠癌。Objective To analyze the relationship between the positive expression of nucleolar spindle-associated protein 1(NUSAP1)and secreted protein 1(DKK1)in colorectal cancer tissues with pathological features,and to explore their impact on patient prognosis.Methods A total of 95 colorectal cancer patients admitted to Luohe Central Hospital from January 2020 to June 2021 were selected as the study subjects.Immunohistochemistry was used to analyze the positive expression of NUSAP1 and DKK1 in colorectal cancer tissues and adjacent tissues.The diagnostic value of NUSAP1 and DKK1 in colorectal cancer was evaluated using receiver operating characteristic(ROC)curves and the area under the curve(AUC).The positive expression levels of NUSAP1 and DKK1 were compared among patients with different pathological features.Kaplan-Meier survival curves were used to analyze the survival outcomes of colorectal cancer patients based on NUSAP1 and DKK1 expression,and COX regression analysis was performed to identify factors influencing patient prognosis.Results The positive expression rate of NUSAP1 in cancer tissues was 78.95%,significantly higher than that in adjacent tissues(13.68%),while the positive expression rate of DKK1 in cancer tissues was 54.74%,significantly lower than that in adjacent tissues(72.63%).Both differences were statistically significant(P<0.05).ROC curve analysis showed that the AUC values for NUSAP1,DKK1,and their combined diagnosis of colorectal cancer were 0.826(95%CI:0.765-0.877),0.589(95%CI:0.516-0.660),and 0.900(95%CI:0.848-0.939),respectively.The combined diagnosis had a higher AUC,with optimal sensitivity and specificity of 89.47%and 90.53%,respectively.Significant differences in NUSAP1 and DKK1 positive expression were observed among patients with different TNM stages,tumor diameters,lymph node metastasis,and distant metastasis(P<0.05).Kaplan-Meier curve analysis revealed that the 3-year survival rate for patients with positive NUSAP1 expression was 60.29%,significantly lower than that of patients with nega
