数字健康技术在基层医疗卫生机构慢性病健康管理的应用——以糖尿病、高血压为例

摘　　要：目的对数字健康技术在基层医疗卫生机构慢性病患者健康管理应用的相关研究进行范围综述。方法检索PubMed、Web of science、Embase、中国知网、万方和维普等数据库中的相关研究,检索时限为建库至2025年2月。结果共纳入19篇文献,数字健康技术用于基层医疗卫生机构慢性病患者管理的主要形式包括社交媒体、健康管理软件、网络平台、智能可穿戴设备;内容包括日常行为管理、健康监测、在线问诊等。数字健康技术可改善患者血糖、血压、糖化血红蛋白等指标,提升患者满意度。结论数字健康技术在基层医疗卫生机构慢性病健康管理中发挥了积极作用。未来应进一步拓展更符合基层实际的数字健康技术形式及内容,推动我国基层医疗卫生服务事业可持续发展。Objective To conduct a scoping review on the studies of the implementation of digital health technology in chronic disease management in primary healthcare institutions.Methods A comprehensive search was conducted across databases including PubMed,Web of Science,Embase,CNKI,Wanfang,and VIP,with the search period from inception to February 2025.Results A total of 19 studies were included.Findings revealed that digital health technology used in primary healthcare institutions mainly comprised social media platforms,health management software,online platforms,and smart wearable devices.Key management areas involved daily behavior management,health monitoring,and telemedicine consultation.Implementation of these technology significantly improved not just clinical outcomes,including blood glucose,blood pressure,and glycated hemoglobin levels,but patient satisfaction as well.Conclusions Digital health technologies demonstrate significant potential for improving chronic disease management in primary healthcare institutions.Future research and implementation should focus on the variety of modalities and functionality of these technologies to align with the needs of primary care in underserved areas,in order to foster sustainable improvements in China’s primary healthcare delivery.
