玻璃体腔注射雷珠单抗联合玻璃体切除术治疗PDR的疗效分析

摘　　要：目的探讨增生性糖尿病视网膜病变治疗中,玻璃体切除术联合玻璃体腔注射雷珠单抗的临床疗效。方法非随机选取2021年1月—2023年6月徐州市贾汪区人民医院收治的增生性糖尿病视网膜病变患者78例为研究对象,按治疗方法不同分为两组,各39例。对照组采取玻璃体切除术治疗、研究组采取玻璃体切除术+玻璃体腔注射雷珠单抗治疗;比较两组最佳矫正视力、眼压、血管内皮生长因子、白细胞介素19水平及并发症发生情况。结果治疗后,研究组最佳矫正视力较对照组高,眼压较对照组低,差异有统计学意义(P均<0.05);研究组血管内皮生长因子及白细胞介素19水平较对照组低,差异有统计学意义(P均<0.05)。研究组并发症发生率为5.13%(2/39),较对照组的20.51%(8/39)低,差异有统计学意义(χ^(2)=4.129,P<0.05)。结论玻璃体切除术+玻璃体腔注射雷珠单抗治疗增生性糖尿病视网膜病变较单纯玻璃体切除术效果显著,可提高患者视力,降低术后眼压,改善血管内皮生长因子、白细胞介素19水平,且能够减少术后并发症。Objective To investigate the clinical effect of vitrectomy combined with intravitreal injection of leizumab in the treatment of proliferative diabetic retinopathy.Methods Seventy-eight proliferative diabetic retinopathy pa-tients admitted to Jiawang District People's Hospital of Xuzhou City from January 2021 to June 2023 were non-randomly selected as the research objects and divided into two groups according to the different treatment methods,with thirty-nine cases in each group.Vitrectomy treatment was implented in control group and vitrectomy+intravitreal injection of leizumab treatment was implented in study group.Compare the optimal corrected visual acuity,intraocular pressure,vascular endothelial growth factor,interleukin 19 levels and complications of two groups.Results After treat-ment,the study group's best corrected visual acuity was higher than that in the control group,and the intraocular pres-sure was lower than that in the control group,the differences were statistically significant(both P<0.05).Vascular en-dothelial growth factor and interleukin 19 levels in the study group were lower than those in the control group,the dif-ferences were statistically significant(both P<0.05).The study group's complication rate was 5.13%(2/39),lower than 20.51%(8/39)in the control group,and the difference was statistically significant(χ^(2)=4.129,P<0.05).Conclusion Vitrectomy+intravitreal injection of leizumab treatment is more effective than Vitrectomy alone in the treatment of pro-liferative diabetic retinopathy,which can improve patients'visual acuity,reduce postoperative intraocular pressure,improve the levels of vascular endothelial growth factor and interleukin 19,and reduce postoperative complications.
