摘　　要：目的:比较不同粘接方案在错(牙合)畸形口腔正畸治疗的效果。方法:选取2020年6月至2023年6月在我院就诊的80例需行口腔正畸的错(牙合)畸形患者,随机分为常规组和改良组(n=40),分别采用直接粘接法和改良式间接粘接法进行治疗。统计治疗后1 m、3 m、6 m时的托槽脱落率,在完成粘接后使用口内扫描仪检测近远中向、颊舌向以及龈(牙合)向粘接严重偏差发生率;统计椅旁操作时间;在治疗前、治疗后6 m时检测牙齿松动度(Tooth mobility,TM)、改良菌斑指数(Modified Plaque index,mPLI);治疗当天、治疗后1 m、治疗后3 m以及治疗后6 m时使用口腔健康影响程度量表(Oral Health Impact Profile,OHIP-14)评估生活质量;统计不良反应。结果:治疗后3、6 m,改良组托槽脱落率低于常规组,粘接严重偏差率低于常规组,椅旁操作时间短于常规组(P<0.05)。治疗6m,mPLI、TM低于常规组,(P<0.05)。治疗1m、3m,改良组OHIP-14评分低于常规组(P<0.05)。结论:改良式间接粘接法能有效减少托槽脱落率、粘接严重偏差发生率,缩短椅旁操作时间。Objective:To compare the effects of different bonding schemes in orthodontic treatment of malocclusion.Method:A total of 80 patients with malocclusion who needed orthodontic treatment in our hospital from June 2020 to June 2023 were randomly divided into a routine group and a modified group(n=40).The direct and modified indirect bonding methods were used for treatment,respectively.The bracket loss rates at 1 month,3 months,and 6 months after treatment were statistically analyzed.After the bonding was completed,the incidence of severe deviation in the mesiodistal,buccolingual,and gingival(occlusal)directions was detected by an intraoral scanner.The chair-side operation time was counted;tooth mobility(TM)and modified plaque index(mPLI)were measured before and 6 months after treatment.Oral Health Impact Profile(OHIP-14)was used to evaluate the quality of life on the day of treatment,1 month,3 months,and 6 months after treatment.Adverse reactions were counted.Results:At 3 and 6 months after treatment,the bracket shedding rate of the modified group was lower than that of the routine group,the serious deviation rate of bonding was lower than that of the routine group,and the chairside operation time was shorter than that of the routine group(P<0.05).After 6 months of treatment,mPLI and TM were lower than those in the routine group(P<0.05).After 1 month and 3 months of treatment,the OHIP-14 score of the modified group was lower than that of the routine group(P<0.05).Conclusion:The modified indirect bonding method can effectively reduce the bracket loss rate,and the incidence of severe bonding deviation,and shorten the chairside operation time.
