CT血管成像、多层螺旋CT在脑梗死患者颈动脉狭窄及侧支循环评估中的价值分析

摘　　要：目的:探究CT血管成像联合多层螺旋CT对脑梗死患者颈动脉狭窄及侧支循环的评估价值。方法:回顾性收集2018年2月~2023年8月期间于本院就诊的60例脑梗死患者的临床资料。患者均以数字减影血管造影作为诊断脑梗死和颈动脉狭窄及侧支循环的金标准。分析比较CT血管成像、多层螺旋CT单独及联合评估颈动脉狭窄、侧支循环的结果,并分析其与数字减影血管造影的一致性。结果:CT血管成像联合多层螺旋CT诊断不同程度颈动脉狭窄检出率及颈动脉粥样硬化斑块结果均显著高于CT血管成像或多层螺旋CT单独诊断(P<0.05)。CT血管成像评估侧支循环一致性Kappa为0.512,多层螺旋CT评估侧支循环一致性Kappa为0.507,两者联合诊断评估侧支循环一致性Kappa为0.796,显著高于单独诊断(P<0.05)。结论:CT血管成像联合多层螺旋CT在脑梗死患者颈动脉狭窄及侧支循环中具有较高的评估价值。Objective:To explore the value of CT angiography combined with multi-slice spiral CT in the assessment of carotid artery stenosis and collateral circulation in patients with cerebral infarction.Methods:The clinical data of 60 patients with cerebral infarction from February 2018 to August 2023 were retrospectively collected.Digital subtraction angiography was used as the gold standard for diagnosing cerebral infarction,carotid stenosis and collateral circulation in all patients.The results of CT angiography and multi-slice spiral CT alone and in combination in the assessment of carotid stenosis and collateral circulation were compared,and the consistency with digital subtraction angiography was analyzed.Results:The detection rate of carotid artery stenosis and carotid atherosclerotic plaque by CT angiography combined with MSCT were significantly higher than that by CT angiography or MSCT alone(P<0.05).The Kappa value of collateral circulation consistency assessed by CT angiography was 0.512,that assessed by multi-slice spiral CT was 0.507,and that assessed by combined diagnosis was 0.796,which was significantly higher than that assessed by single diagnosis(P<0.05).Conclusion:CT angiography combined with multi-slice spiral CT has high value in the evaluation of carotid artery stenosis and collateral circulation in patients with cerebral infarction.
