旋金降逆汤辅助富马酸伏诺拉生片对反流性食管炎患者中医证候积分及炎症反应的影响

摘　　要：目的:研究旋金降逆汤辅助富马酸伏诺拉生片对反流性食管炎(Reflux Esophagitis,RE)患者中医证候积分及炎症反应的影响。方法:采用随机数字表法将我院2021年6月至2024年2月期间收治的114例RE患者进行随机分组,对照组57例患者采用富马酸伏诺拉生片治疗;研究组57例患者采用旋金降逆汤联合富马酸伏诺拉生片治疗。治疗8 w后,分析比较两组的临床疗效、治疗前后中医证候积分、食管动力学指标、血清炎症介质水平以及不良反应发生率。结果:研究组临床总有效率高于对照组(P<0.05)。治疗后,两组的反食、胃脘疼痛、灼烧感、反酸各项中医证候积分均低于治疗前,且研究组低于对照组(P<0.05)。治疗后,两组上括约肌静息压、下括约肌静息压均高于治疗前,且研究组高于对照组(P<0.05)。治疗后,两组血清超敏C反应蛋白、白细胞介素-13、环氧化酶、核因子-kB水平均低于治疗前,且研究组低于对照组(P<0.05)。两组的不良反应发生率无显著差异(P>0.05)。结论:采用旋金降逆汤辅助富马酸伏诺拉生片治疗RE患者,能提高治疗效果,改善临床症状和食管动力学,减轻炎症,且安全性较好。Objective:To investigate the effect of Xuanjin Jiangni decoction-assisted vonoprazan fumarate tablets on the TCM syndrome score and inflammatory response in patients with reflux esophagitis(RE).Methods:A random number table method was used to randomly divide 114 patients with RE admitted to our hospital from June 2021 to February 2024 into groups.The control group of 57 patients was treated with vonoprazan fumarate tablets;the study group of 57 patients was treated with Xuanjin Jiangni decoction combined with vonoprazan fumarate tablets.After 8 weeks of treatment,the clinical efficacy of the two groups was analyzed and compared,as well as the TCM syndrome scores,esophageal dynamics indicators,serum inflammatory mediator levels before and after treatment,and adverse reaction rate between the two groups was compared.Results:The total clinical effective rate of the study group was higher than that of the control group(P<0.05).After treatment,the scores of the two groups for the traditional Chinese medicine syndromes of regurgitation,epigastric pain,burning sensation,and acid reflux were all lower than before treatment,and the study group was lower than the control group(P<0.05).After treatment,the resting pressure of the upper and lower sphincters in both groups was higher than before treatment,and the study group was higher than the control group(P<0.05).After treatment,the levels of serum high-sensitivity C-reactive protein,interleukin-13,cyclooxygenase,and nuclear factor-kB in both groups were lower than before treatment,and the study group was lower than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The use of Xuanjin Jiangni decoction-assisted vonoprazan fumarate tablets in the treatment of RE patients can improve the therapeutic effect,improve clinical symptoms and esophageal dynamics,reduce inflammation,and has good safety.
