摘 要:目的:探究乳腺微小结节的良恶性采用高频超声血流动力学参数诊断的价值。方法:收集2023年1月—2024年6月期间在梅州市梅县区中医医院接受诊疗的108例乳腺结节病变患者,作为观察对象。均进行高频超声诊断,“金标准”为手术病理检查结果。分析高频超声诊断乳腺微小结节的良恶性结果及效能;比较良恶性结节的血流动力学参数[阻力指数(RI)、血流收缩期峰值流速(PSV)];比较良恶性结节患者血流分级情况。结果:108例乳腺结节病变患者共检出122个结节;经手术病理诊断,31个恶性结节、91个良性结节;经高频超声诊断,29个恶性结节、93个良性结节,且诊断敏感度为83.87%(26/31)、特异度为96.70%(88/91)、准确度为93.44%(114/122)。阳性预测值为89.66%(26/29)、阴性预测值为94.62%(88/93)。相比良性乳腺微小结节,恶性乳腺微小结节的血流动力学参数RI、PSV数值更高(P<0.05)。相比良性乳腺微小结节,恶性乳腺微小结节的血流分级更高(P<0.05)。结论:高频超声诊断乳腺微小结节良恶性的效能较高,且结合血流动力学参数RI与PSV、血流分级信号能进一步鉴别乳腺结节的良恶性,值得推广应用。Objective:To explore the diagnostic value of high-frequency ultrasound hemodynamic parameters for benign and malignant breast micronodules.Methods:108 patients with breast nodular lesions who received diagnosis and treatment in Meixian District Hospital of Traditional Chinese Medicine in Meizhou City from January 2023 to June 2024 were collected as observation objects.High-frequency ultrasound diagnosis was performed in all patients,and the“gold standard”was the results of pathological examination.To analyze the benign and malignant results and efficacy of high-frequency ultrasound in the diagnosis of breast micronodules.The hemodynamic parameters(resistance index(RI),peak systolic flow velocity(PSV))of benign and malignant nodules were compared.The blood flow grading of benign and malignant nodules was compared.Results:A total of 122 nodules were detected in 108 patients.There were 31 malignant nodules and 91 benign nodules diagnosed by surgical pathology.29 malignant nodules and 93 benign nodules were diagnosed by high-frequency ultrasound,and the diagnostic sensitivity was 83.87%(26/31),specificity was 96.70%(88/91),and accuracy was 93.44%(114/122).The positive predictive value was 89.66%(26/29)and the negative predictive value was 94.62%(88/93).The RI and PSV values of malignant breast small nodules were higher than those of benign breast small nodules(P<0.05).Compared with benign breast small nodules,the blood flow grade of malignant breast small nodules was higher(P<0.05).Conclusion:High frequency ultrasound is effective in the diagnosis of benign and malignant breast micro-nodules,and combined with hemodynamic parameters RI,PSV and blood flow classification signal can further distinguish benign and malignant breast nodules,which is worthy of promotion and application.
