摘  要:目的利用生物信息学方法结合实验分析CHI3L1在头颈部鳞状细胞癌及主要亚型喉鳞状细胞癌中的差异表达,探讨其表达与患者预后之间的关系。方法利用在线数据库分析CHI3L1在头颈部鳞状细胞癌组织中的差异表达,探索其与头颈部鳞状细胞癌患者预后之间的关系,同时从头颈部鳞状细胞癌差异表达基因中筛选出CHI3L1的共表达基因,根据富集分析探索CHI3L1可能参与的生物过程。通过Western-Blot、RT-PCR和免疫组化法验证CHI3L1在喉鳞状细胞癌中表达情况,并从Kaplan-Meier生存曲线、单因素风险模型分析及多因素风险比例模型中验证CHI3L1与喉鳞状细胞癌患者预后间的关系。结果CHI3L1在头颈部鳞状细胞癌组织中表达明显升高(P=0.029),对头颈部鳞状细胞癌具有一定的诊断意义(AUC=0.599);富集分析结果提示,CHI3L1可能与细胞分泌、原生质膜及其功能、蛋白的聚合活动有关(P<0.05);其作用机制可能与Apelin通路、Rap1通路等通路有关(P<0.05)。Western-Blot及PCR实验结果提示,CHI3L1在喉鳞癌组织中表达水平升高(P<0.01)。免疫组化检测结果表明,喉鳞状细胞癌组织中CHI3L1蛋白的表达例数明显较多(P=0.007);喉鳞状细胞癌患者预后情况与患者T分期、淋巴结转移及CHI3L1表达相关(P<0.05),且T分期可作为患者独立预后风险因素存在(P=0.009)。结论CHI3L1在喉鳞状细胞癌组织中高表达,可能通过Apelin、Rap1等因子通路参与生物学功能,与喉鳞状细胞癌患者的预后相关。Objective To analyze the differential expression of CHI3L1 in head and neck squamous-cell carcinoma(HNSCC)and its main subtypes especially laryngeal squamous-cell carcinoma(LSCC)and explore the correlation between the expression and the prognosis of the patients by bioinformatics methods combined with experiments.Methods Online database was used to analyze the differential expression of CHI3L1 in HNSCC and found out the correlation of the expression of CHI3L1 and the prognosis of patients with HNSCC.The co-expression genes of CHI3L1 were screened from the differentially expressed genes of HNSCC and enrichment analysis was performed to explore the biological process.The expression of CHI3L1 in laryngeal squamous cell carcinoma was confirmed by Western-Blot,RT-PCR and Immunohistochemistry.Meanwhile,the relationship between CHI3L1 and the prognosis of LSCC patients was verified by Kaplan-Meier survival curve,univariate risk model analysis and multivariate risk proportion model.Results The expression of CHI3L1 was up-regulated in the tissues of HNSCC(P=0.029).It had diagnostic significance for the HNSCC(AUC=0.599).The expression of CHI3L1 may be related to cell secretion,protoplasma membrane and its function,and protein activity(P<0.05).The mechanism of CHI3L1 may be related to the pathway of Apelin and Rap1(P<0.05).CHI3L1 was up-regulated in the tissues of LSCC in the Western-Blot and PCR experiment(P<0.01).The results of immunohistochemical analysis showed that the expression of CHI3L1 protein in laryngeal squamous cell carcinoma tissues was significantly higher(P=0.007).The prognosis of patients with LSCC might be related to T stage,lymph node metastasis and CHI3L1 expression(P<0.05),and T stage existed as an independent prognostic risk factor for patients(P=0.009).Conclusion The expression of CHI3L1 in LSCC is up-regulated and CHI3L1 may participate in biological functions through Apelin,Rap1 and other factor pathways and be related with the prognosis of patients with LSCC.
