术后应用酮咯酸氨丁三醇镇痛与亚甲蓝注射液配合对痔疮手术患者疼痛状况的改善作用分析

摘　　要：目的探究术后应用酮咯酸氨丁三醇镇痛与亚甲蓝注射液配合对痔疮手术患者疼痛状况的改善作用。方法非随机选取山东省郯城县中医医院于2020年2月—2023年2月收治的80例行痔疮手术的患者为研究对象。利用术后采取的镇痛措施不同分为对照组与观察组,各40例。对照组注射亚甲蓝溶液,观察组在对照组基础上注射酮咯酸氨丁三醇。对比两组疼痛及舒适程度评分、炎性因子水平及不良反应发生情况。结果治疗后观察组疼痛度(2.56±1.06)分低于对照组(4.21±1.34)分,舒适度(3.51±0.41)分高于对照组(2.13±0.78)分,差异有统计学意义(t=6.108、9.905,P均<0.05)。治疗后观察组肿瘤坏死因子、白细胞介素1β、白细胞介素8水平均低于对照组,差异有统计学意义(P均<0.05)。观察组在尿潴留、口干、头晕头痛、胃肠道不适这些不良反应方面的总发生率为2.50%(1/40),低于对照组的15.00%(6/40),差异有统计学意义(χ^(2)=3.914,P<0.05)。结论针对痔疮手术患者,在术后采用酮咯酸氨丁三醇配合亚甲蓝注射液镇痛有助于缓解患者的疼痛程度、改善舒适度,且炎性因子水平更低,不良反应更少。Objective To explore the effect of analgesia with ketorolac tropanol and methylene blue injection on the improvement of pain in patients undergoing hemorrhoid surgery.Methods Eighty patients undergoing hemorrhoid surgery in Tancheng County Traditional Chinese Medicine Hospital of Shandong Province from February 2020 to February 2023 were non-randomLy selected as the study objects.The patients were divided into control group and observation group with forty cases in each group.The control group was injected with methylene blue solution,and the observation group was injected with ketorolac trobutamine based on the control group.The level of pain and comfort scores,inflammatory factors and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the pain score of the observation group was 2.56±1.06,which was lower than 4.21±1.34 of the control group,and the comfort score was 3.51±0.41,which was higher than 2.13±0.78 of the control group,the differences were statistically significant(t=6.108,9.905,both P<0.05).After treatment,the levels of tumor necrosis factor,interleukin-1βand interleukin-8 in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The total incidence of adverse reactions such as urinary retention,dry mouth,dizziness,headache and gastrointestinal discomfort in the observation group was 2.50%(1/40),which was lower than 15.00%(6/40)in the control group,and the difference was statistically significant(χ^(2)=3.914,P<0.05).Conclusion For patients undergoing hemorrhoid surgery,the use of ketorolac tropanol combined with methylene blue injection for analgesia after surgery can help alleviate the pain degree and improve comfort,and the level of inflammatory factors is lower,and the adverse reactions are less.
