摘　　要：目的:核算并分析2022年吉林省老年人群的治疗费用,从卫生费用核算的角度探索老年人群治疗费用的影响因素,为老年人群疾病经济负担减轻提供合理参考。方法:通过多阶段分层抽样,抽取吉林省916家医疗卫生机构,建立2022年吉林省老年人群个案数据库。在“卫生费用核算体系2011”框架下,采用“自上而下”的分摊方法,核算吉林省老年人群治疗总费用。建立Lasso回归与分层回归模型,研究老年人群治疗费用的影响因素。结果:2022年吉林省老年人群治疗费用为213.28亿元,占总治疗费的32.44%。年龄、性别、参保类型、住院天数、机构类型、机构级别和治疗服务对老年人群的治疗费用具有统计学意义(P<0.05)。结论:吉林省老年人群治疗费用占总人群治疗费用比重较大,看病负担重,应从重点病种出发进行防控,优化分级诊疗制度,促进医疗资源的合理配置。Objective:This study calculated and analyzed the elderly treatment cost from Jilin Province in 2022.From the perspective of health cost accounting,the study explored the factors of the elderly treatment cost thus offering a reasonable reference to the ease of their financial burden.Methods:Using multi-stage stratified sampling,916 medical and health institutions in Jilin province were selected to establish the case database of the elderly in Jilin province in 2022.Under the framework of"SHA 2011",the total treatment cost of the elderly population in Jilin province was calculated with the method of"top-down"allocation.Lasso regression and hierarchical regression models were established to study the factors affecting the treatment cost of the elderly.Results:The elderly cost was 21.328 billion covering 32.44%in total from Jilin province in 2022.Age,gender,insurance type,hospitalization days,institution type,institution level and treatment service were statistically significant for treatment costs in the elderly(P<0.05).Conclusion:The proportion of the elderly treatment cost is larger than the total population's counterpart and the treatment cost burden is heavy in Jilin Province.So prevention and control must be made from the major disease to upgrade the system of tiered medical services and facilitate the reasonable allocation of medical resources.
