血清HE4联合CA125检验对上皮性卵巢癌的诊断价值研究

摘　　要：目的分析血清人附睾蛋白4(HE4)联合癌抗原125(CA125)检验对上皮性卵巢癌(EOC)的诊断价值。方法84例卵巢病变患者,根据病理结果将40例EOC患者作为研究组,44例卵巢良性疾病患者作为对照组。两组均接受血清HE4联合CA125检验。比较两组血清HE4、CA125水平,血清HE4、CA125单独检验与联合检验对EOC的诊断效能(诊断特异度、敏感度、阴性预测值、阳性预测值、准确度)。结果研究组血清HE4、CA125水平分别为(258.65±24.89)pmol/L、(320.60±25.02)U/ml,较对照组的(38.60±8.79)pmol/L、(30.32±4.02)U/ml高(P<0.05)。血清HE4检出真阳性31例(77.50%),CA125检出真阳性28例(70.00%),HE4+CA125检出真阳性38例(95.00%)。HE4+CA125联合检验对EOC的诊断特异度、敏感度、阴性预测值、阳性预测值、准确度均高于血清HE4、CA125单独检验(P<0.05)。结论EOC患者血清HE4与CA125表达显著上升,二者联合检验能够有效提高对EOC的诊断效能,为临床诊疗提供重要的参考信息。Objective To analyze the diagnostic value of serum human epididymis protein 4(HE4)combined with cancer antigen 125(CA125)test in epithelial ovarian cancer(EOC).Methods Among 84 patients with ovarian lesions,40 patients with epithelial ovarian cancer were taken as study group and 44 patients with benign ovarian diseases were taken as control group based on pathological results.Both groups received serum HE4 combined with CA125 test.Comparison was made on serum HE4 and CA125 levels between the two groups,and the diagnostic efficacy(diagnostic specificity,sensitivity,negative predictive value,positive predictive value,accuracy)of serum HE4 and CA125 alone and combined tests for epithelial ovarian cancer.Results Serum levels of HE4 and CA125 in the study group were(258.65±24.89)pmol/L and(320.60±25.02)U/ml,which were higher than(38.60±8.79)pmol/L and(30.32±4.02)U/ml in the control group(P<0.05).Serum HE4 detected true positives in 31 cases (77.50%), CA125 detected true positives in 28 cases (70.00%), and HE4+CA125 detected true positives in 38 cases (95.00%). The diagnostic specificity, sensitivity, negative predictive value, positive predictive value, and accuracy of the combined test for epithelial ovarian cancer were higher than those of serum HE4 and CA125 alone (P<0.05). Conclusion Serum expressions of HE4 and CA125 increased significantly in patients with epithelial ovarian cancer, and the combined tests of the two can effectively improve the diagnostic efficiency of epithelial ovarian cancer and provide important reference information for clinical diagnosis and treatment.
