摘　　要：目的 探讨草酸艾司西酞普兰联合氟哌噻吨美利曲辛片在脑卒中后抑郁(PSD)患者中的应用效果。方法 选取2021年12月—2023年12月成武县人民医院收治的106例PSD患者为研究对象,按随机数字表法将其分为对照组和观察组,每组53例。对照组采用氟哌噻吨美利曲辛片治疗,观察组在对照组基础上采用草酸艾司西酞普兰治疗,两组均持续治疗2个月。比较两组的临床疗效、抑郁程度、神经递质水平、不良反应发生情况。结果 观察组治疗总有效率为96.23%,高于对照组的73.58%,差异有统计学意义(P<0.05)。治疗后,观察组汉密尔顿抑郁量表评分低于对照组,差异有统计学意义(P<0.05);观察组5-羟色胺、去甲肾上腺素、多巴胺水平均高于对照组,组间差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 草酸艾司西酞普兰联合氟哌噻吨美利曲辛片治疗PSD的疗效显著,可减轻患者抑郁症状,调节神经递质水平,且用药安全。Objective To explore the application effect of escitalopram oxalate combined with flupentixol and melitracen tablets in patients with post-stroke depression(PSD).Methods A total of 106 patients with PSD admitted to Chengwu County People’s Hospital from December 2021 to December 2023 were selected as the research objects,they were divided into a control group and an observation group according to the random number table method,with 53 cases in each group.The control group was treated with flupentixol and melitracen tablets,and the observation group was treated with escitalopram oxalate on the basis of the control group,both groups were continuously treated for 2 months.The clinical efficacy,degree of depression,neurotransmitter levels,and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was 96.23%,which was higher than 73.58%in the control group,and the difference was statistically significant(P<0.05).After treatment,the score of the Hamilton Depression Scale in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05);the levels of serotonin,norepinephrine,and dopamine in the observation group were all higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment of PSD with escitalopram oxalate combined with flupentixol and melitracen tablets has a remarkable curative effect.It can relieve the depressive symptoms of patients,regulate the levels of neurotransmitters,and the medication is safe.
