摘 要:目的探讨运动疗法联合三维步态训练治疗对脑卒中后遗症患者肢体运动功能的影响。方法选取2023年7月—2024年9月北京积水潭医院贵州医院收治的60例脑卒中后遗症患者为研究对象,按随机数字表法将其分为对照组和观察组,各30例。对照组采用运动疗法,观察组在对照组基础上联合三维步态训练。比较两组的平衡能力、肢体运动功能、步态能力、步行速度、日常生活能力及生活质量。结果治疗后,观察组Berg平衡量表评分为(49.57±3.34)分,高于对照组的(42.89±4.45)分,差异有统计学意义(P<0.05);观察组Fugl-Meyer运动功能量表中上肢功能与下肢功能、Tinetti步态评估量表、Barthel指数量表、脑卒中生活质量量表评分均高于对照组,10 m最大步行速度快于对照组,组间差异有统计学意义(P<0.05)。结论脑卒中后遗症患者采用运动疗法联合三维步态训练治疗,可改善平衡能力与步态能力,提升肢体运动功能,提高生活质量,值得推广。Objective To investigate the effect of exercise therapy combined with three-dimensional gait training on limb motor function in patients with sequelae of stroke.Methods A total of 60 patients with sequelae of stroke admitted to Beijing Jishuitan Hospital Guizhou Hospital from July 2023 to September 2024 were selected as the research objects,according to the random number table method,they were divided into a control group and an observation group,with 30 cases in each group.The control group was treated with exercise therapy,and the observation group was treated with three-dimensional gait training on the basis of the control group.The balance ability,limb motor function,gait ability,walking speed,activity of daily living and quality of life were compared between the two groups.Results After treatment,the Berg Balance Scale score of the observation group was(49.57±3.34)points,which was higher than(42.89±4.45)points of the control group,and the difference was statistically significant(P<0.05).The scores of upper limb function and lower limb function of Fugl-Meyer Assessment Scale,Tinetti Gait Analysis Scale,Barthel Index and Stroke-Specific Quality of Life Scale in the observation group were higher than those in the control group,and the 10-Meter Maximum Walk Speed was faster than that in the control group,and the differences between the two groups were statistically significant(P<0.05).Conclusion Exercise therapy combined with three-dimensional gait training for patients with sequelae of stroke can improve the balance ability and gait ability,improve limb motor function,and improve the quality of life,which is worthy of promotion.
