摘　　要：目的探究氯吡格雷联合阿司匹林治疗对急性脑梗死(ACI)患者血液流变学指标、认知功能的影响。方法回顾性分析2021年2月—2023年2月新泰市人民医院收治的92例ACI患者的临床资料,按照治疗方法的不同将其分为对照组和观察组,各46例。对照组采用阿司匹林治疗,观察组在对照组基础上联合氯吡格雷治疗,两组均持续治疗2周。对比两组患者的血液流变学指标、神经功能、认知功能及不良反应发生情况。结果治疗后,观察组血小板聚集率、血浆黏度、纤维蛋白原、红细胞压积水平均低于对照组,组间差异有统计学意义(P<0.05);观察组美国国立卫生研究院卒中量表评分为(10.08±1.61)分,低于对照组的(14.01±2.71)分,蒙特利尔认知评估量表评分为(26.19±1.05),高于对照组的(24.81±3.42)分,组间差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论氯呲格雷联合阿司匹林治疗ACI患者可有效促进其血液流变学指标、神经功能、认知功能的改善,且不良反应较少,值得推广。Objective To investigate the effect of clopidogrel combined with aspirin on blood rheological indicators and cognitive function in patients with acute cerebral infarction(ACI).Methods The clinical data of 92 patients with ACI admitted to Xintai People’s Hospital from February 2021 to February 2023 were retrospectively analyzed,according to the different treatment methods,they were divided into a control group and an observation group,with 46 cases in each group.The control group was treated with aspirin,and the observation group was treated with clopidogrel on the basis of the control group,both groups were treated for 2 weeks.The blood rheological indicators,neurological function,cognitive function and occurrence of adverse reactions of the two groups were compared.Results After treatment,the platelet aggregation rate,plasma viscosity,fibrinogen,and hematocrit in the observation group were lower than those in the control group,and the differences between the two groups were statistically significant(P<0.05);the National Institutes of Health Stroke Scale score of the observation group was(10.08±1.61)points,which was lower than(14.01±2.71)points of the control group,and the Montreal Cognitive Assessment Scale score was(26.19±1.05)points,which was higher than(24.81±3.42)points of the control group,the differences between the two groups were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Clopidogrel combined with aspirin in the treatment of patients with ACI can effectively promote the improvement of blood rheological indicators,neurological function and cognitive function,and has fewer adverse reactions,which is worthy of promotion.
