血糖变异性对急性缺血性脑卒中患者早期神经功能恶化的影响研究进展

摘　　要：急性缺血性脑卒中(acute ischemic stroke,AIS)患者发生早期神经功能恶化(early neurological deterioration,END),可能与血糖变异性(glucose variability,GV)的严重程度有一定相关性。一些研究表明,通过应用动态血糖监测技术,收集急性缺血性脑卒中患者的血糖波动指标,包括血糖标准差(SD)、平均血糖波动范围(MAGE)和血糖变异系数(CV)等,以及在院期间的静态参数,如空腹血糖(FBG)和糖化血红蛋白(HbA1c)等,可以发现患者在发病初期的血糖波动幅度较大。血糖变异性与早期神经功能恶化的出现可能存在正向关联,这一关联在血糖超过临界值时较为明显。如果将血糖变异性作为急性缺血性脑卒中患者发生早期神经功能恶化的一种生物标志物,或许可以指导临床医生的决策,也可能为预防和控制脑卒中带来的严重后果提供一定参考。为此,对近年来的相关研究进展作一综述,旨在提高人们对血糖变异性的认识,强调其在临床工作中的作用,为降低脑卒中患者病情提供一定依据。Early neurological deterioration(END)in patients with acute ischemic stroke(AIS)may be somewhat correlated with the severity of glucose variability(GV).Some studies have shown that by using continuous glucose monitoring technology to collect the indicators of glucose fl uctuations in patients with AIS,including the standard deviation of blood glucose(SD),the mean amplitude of glycemic excursions(MAGE),and the coefficient of variation of blood glucose(CV),as well as the static parameters during hospitalization,such as fasting blood glucose(FBG)and glycated hemoglobin(HbA1c),it can be found that patients have a large amplitude of glucose fl uctuations in the early stage of the onset of the disease.There may be a positive correlation between glucose variability and the occurrence of early neurological deterioration,and this correlation is more obvious when the blood glucose exceeds the critical value.If glucose variability can be used as a biomarker for the occurrence of early neurological deterioration in patients with AIS,it may guide the decision-making of clinicians and may also provide some reference for the prevention and control of the serious consequences brought about by stroke.Therefore,this article reviews the relevant research progress in recent years,aiming to improve people’s understanding of glucose variability,emphasize its role in clinical work,and provide a certain basis for reducing the progression of the disease in stroke patients.
