摘 要:目的探讨溃疡性结肠炎(UC)患者粪便细菌、粪钙卫蛋白(FC)、全身免疫炎症指数(SII)、D-二聚体(D-D)水平及疾病严重程度的相关性。方法选择西京医院2018年10月至2019年12月接诊的UC患者115例为研究对象,根据疾病严重程度分为轻度组(n=46)、中度组(n=39)和重度组(n=30)。比较三组患者的粪便细菌分布情况、FC、SII、D-D水平,并比较不同预后患者的粪便细菌分布情况、FC、SII、D-D水平,采用Pearson相关分析法分析UC患者粪便细菌、FC、SII、D-D水平与疾病严重程度及预后的相关性。结果重度组UC患者肠球菌、大肠杆菌、拟杆菌水平均高于中度组、轻度组UC患者,乳酸杆菌、双歧杆菌明显低于中度组、轻度组UC患者(均P<0.05);中度组UC患者肠球菌、大肠杆菌、拟杆菌水平均高于轻度组UC患者,乳酸杆菌、双歧杆菌明显低于轻度组UC患者(均P<0.05)。重度组UC患者FC、SII、D-D水平均高于中度组、轻度组UC患者,中度组UC患者FC、SII、D-D水平均高于轻度组UC患者,差异均有统计学意义(P<0.05)。门诊随访6个月后,115例UC患者中预后良好88例,预后不良27例。预后不良组UC患者肠球菌、大肠杆菌、拟杆菌、FC、SII、D-D水平均高于预后良好组UC患者,乳酸杆菌、双歧杆菌明显低于预后良好组UC患者,差异均有统计学意义(P<0.05)。Pearson相关分析显示,UC患者肠球菌、大肠杆菌、拟杆菌、FC、SII、D-D水平与疾病严重程度均呈正相关,与预后呈负相关(P<0.05);乳酸杆菌、双歧杆菌水平与疾病严重程度均呈负相关,与预后呈正相关(P<0.05)。结论UC患者粪便细菌失调、FC、SII、D-D水平显著升高,与疾病严重程度密切相关,临床检测这些指标有助于精准评估UC患者的病情和治疗效果。Objective To investigate the correlation of fecal bacteria,fecal calprotectin(FC),systemic immunoinflammatory index(SII),D-dimer(D-D)levels and disease severity in patients with ulcerative colitis(UC).Methods A total of 115 UC patients admitted to Xijing Hospital from October 2018 to December 2019 were selected as the study objects,and they were divided into the mild group(n=46),the moderate group(n=39),and the severe group(n=30)according to the severity of the disease.The distribution of fecal bacteria,FC,SII,and D-D levels of patients in the three groups were compared,and the distribution of fecal bacteria,FC,SII and D-D levels in patients with different prognosis were compared.Pearson correlation analysis was used to analyze the correlation between fecal bacteria,FC,SII and D-D levels and disease severity and prognosis in UC patients.Results The levels of Enterococcus,Escherichia coli and Bacteroides in the severe UC group were higher than those in the moderate and mild UC groups;the levels of Lactobacillus and Bifidobacteria were significantly lower than those in the moderate and mild UC groups,the levels of Enterococcus,Escherichia coli and Bacteroides in the moderate UC group were higher than those in the mild UC group;the levels of Lactobacillus and Bifidobacteria in the moderate UC group were significantly lower than those in the mild UC group,with statistically significant differences(P<0.05);the levels of FC,SII and D-D in the severe UC group were higher than those in the moderate and mild UC groups,and the levels of FC,SII and D-D in the moderate UC group were higher than those in the mild UC group,with statistically significant differences(P<0.05).After 6 months of outpatient follow-up of 115 UC patients,88 cases had good prognosis and 27 cases had poor prognosis.The levels of Enterococcus,Escherichia coli,Bacteroides,FC,SII and D-D in UC patients with poor prognosis were higher than those in UC patients with good prognosis,while Lactobacillus and Bifidobacterium were significantly lower than those in
