五龙还少汤联合多奈哌齐片治疗脾肾亏虚型缺血性卒中后认知障碍非痴呆的疗效观察

摘　　要：目的 观察五龙还少汤联合多奈哌齐片对脾肾亏虚型缺血性卒中后认知障碍非痴呆(PSCIND)的临床疗效。方法 选取60例辨证为脾肾亏虚型的缺血性PSCIND患者,按照随机数字表法分为对照组(多奈哌齐片)和联合组(五龙还少汤+多奈哌齐片),各30例,共治疗8周。通过使用简易精神状态检查量表(MMSE)、蒙特利尔认知评估量表(MoCA)、日常生活自理能力量表(ADL-R)和中医证候积分量表对两组病人治疗前后的情况进行评估分析。结果 联合组较对照组能更有效提高患者的MMSE、MoCA评分(P <0.05),降低ADL-R评分(P <0.05)、中医证候积分(P <0.01)。结论 在服用多奈哌齐片的基础上,给予自拟五龙还少汤治疗脾肾亏虚型缺血性PSCIND患者可增强疗效,更好地改善临床症状、认知功能和生活质量,且安全性较高。Objective To observe the clinical efficacy of Wulong Huanshao Decoction combined with donepezil tablets in the treatment of post-stroke cognitive impairment with no dementia(PSCIND)of spleen and kidney deficiency syndrome.Methods Sixty patients with ischemic PSCIND diagnosed as spleen and kidney deficiency syndrome were selected and randomly divided into two groups:a control group(donepezil tablets)and a combination group(Wulong Huanshao Decoction+donepezil tablets)according to a random number table,with 30 cases in each group.Treatment was administered for 8 weeks.The Mini-Mental State Examination(MMSE),Montreal Cognitive Assessment (MoCA), Activity of Daily Living (ADL-R), and traditional Chinese medicine (TCM) syndrome integralscale were used to evaluate the conditions of the two groups before and after treatment. ResultsCompared with the control group, the combination group significantly improved MMSE and MoCAscores (P < 0.05), reduced ADL-R scores (P < 0.05), and decreased TCM syndrome scores (P < 0.01).Conclusion On the basis of donepezil tablets, adding self-formulated Wulong Huanshao Decoctionenhances therapeutic efficacy, improves clinical symptoms, cognitive function, and quality of life inpatients with ischemic PSCIND of spleen and kidney deficiency syndrome, with high safety.
