摘  要:胰腺导管腺癌是一种恶性程度极高的胃肠道肿瘤。目前手术治疗仍被认为是目前唯一有可能治愈胰腺导管腺癌的方式,但多数患者在确诊胰腺导管腺癌时因局部进展或远处转移而失去手术机会,且接受手术的患者多在5年内死于局部复发或远处转移,因此需探索更有效的治疗方案。神经周围浸润是胰腺导管腺癌的显著特征之一,由神经浸润导致的神经周微转移灶在术中难以发现,而残留的病灶则会导致顽固性神经痛、局部病灶复发及远处转移。研究显示,胰腺导管腺癌神经浸润在胰腺导管腺癌发生发展中发挥重要作用,且受到复杂的分子生物学机制调控。进一步研究其发生及其影响胰腺导管腺癌进展的机制,并针对性地进行干预则可能为胰腺导管腺癌神经浸润患者提供针对性治疗方法。本文阐述了胰腺导管腺癌神经浸润在胰腺导管腺癌中的研究现状,并展望其临床应用前景。Pancreatic ductal adenocarcinoma represents a gastrointestinal malignancy characterized by an exceptionally high degree of aggressiveness.To date,surgical intervention remains the sole modality regarded as potentially curative for this disease.Nonetheless,the majority of patients are precluded from undergoing surgery at diagnosis due to locally advanced disease or distant metastasis.Additionally,a substantial proportion of patients who do undergo surgical resection succumb to local recurrence or distant metastasis within a five-year period.Consequently,there is an urgent need to delve into more efficacious therapeutic alternatives.Perineural invasion stands out as a distinctive hallmark of pancreatic ductal adenocarcinoma.The perineural micro-metastases arising from neural infiltration pose significant challenges in intraoperative detection,and residual foci can give rise to persistent neuropathic pain,local recurrence,and distant metastasis.Accumulating evidence suggests that neural infiltration plays a pivotal role in the pathogenesis and progression of pancreatic ductal adenocarcinoma,governed by intricate molecular biological mechanisms.Further elucidation of its occurrence and the underlying mechanisms by which it influences pancreatic ductal adenocarcinoma progression,coupled with targeted interventions,holds the promise of tailored treatment strategies for patients with neural infiltration in pancreatic ductal adenocarcinoma.This article provides an in-depth overview of the current research landscape concerning neural infiltration in pancreatic ductal adenocarcinoma and delineates the potential clinical applications of these findings.
