乳腺外科整复术联合乳腺癌保乳根治术治疗乳腺癌的临床效果

摘　　要：目的 分析乳腺外科整复术联合乳腺癌保乳根治术治疗乳腺癌的临床效果。方法 76例乳腺癌患者,随机分为对照组(n=38)、研究组(n=38)。对照组接受乳腺癌保乳根治术治疗,研究组接受乳腺癌保乳根治术+乳腺外科整复术治疗。比较两组手术相关指标、住院时间、乳房美学效果、并发症发生率及创伤反应指标[肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-4(IL-4)、白细胞介素6(IL-6)]。结果 研究组患者手术时间(151.36±16.28)min长于对照组的(134.69±15.25)min,住院时间(10.23±3.05)d短于对照组的(15.96±3.24)d,术中出血量(114.63±23.28)ml、术后引流量(158.36±34.28)ml少于对照组的(184.23±30.25)、(234.25±37.25)ml(P<0.05)。研究组患者乳房美学优良率78.95%高于对照组的52.63%(P<0.05)。研究组患者并发症发生率5.26%低于对照组的21.05%(P<0.05)。研究组患者术后7 d的TNF-α(56.85±3.87)ng/ml、hs-CRP(2.65±1.17)mg/L、IL-4(69.46±3.24)pg/ml、IL-6(16.25±3.11)ng/L低于对照组的(62.96±3.25)ng/ml、(4.36±1.14)mg/L、(76.54±3.96)pg/ml、(21.13±3.06)ng/L(P<0.05)。结论 对乳腺癌患者开展乳腺癌保乳根治术联合乳腺外科整复术,可改善手术相关指标、促进术后康复,并提高乳房美学效果、降低并发症发生率,缓解创伤反应。Objective To analyze the clinical effect of breast plastic surgery combined with breast conserving radical surgery for the treatment of breast cancer.Methods A total of 76 cases of breast cancer patients were randomly divided into control group(n=38) and study group(n=38).The control group received breast conserving radical surgery,and the study group received breast conserving radical surgery + breast plastic surgery.Comparison of surgery-related index,duration of hospitalization,breast aesthetic effect,incidence of complications and the index of trauma response [tumor necrosis factor-α(TNF-α),hypersensitive C-reactive protein(hs-CRP),interleukin-4(IL-4),interleukin-6(IL-6)] between the two groups.Results The study group had a longer operation time of(151.36±16.28) min than(134.69±15.25) min in the control group;the study group had a shorter duration of hospitalization of(10.23±3.05) d than(15.96±3.24) d in the control group;the study group had intraoperative blood loss of(114.63±23.28) ml and postoperative drainage volume of(158.36±34.28) ml,which were less than(184.23±30.25) and(234.25±37.25) ml in the control group(P<0.05).The rate of breast aesthetic effect in the study group was 78.95%,which was higher than 52.63% in the control group(P<0.05).The complication rate was 5.26% in the study group,which was lower than 21.05% in the control group(P<0.05).At 7 d after operation,the study group had TNF-α of(56.85±3.87) ng/ml,hs-CRP of(2.65±1.17) mg/L,IL-4of(69.46±3.24) pg/ml,and IL-6 of(16.25±3.11) ng/L,which were lower than(62.96±3.25) ng/ml,(4.36±1.14) mg/L,(76.54±3.96) pg/ml,and(21.13±3.06) ng/L in the control group(P<0.05).Conclusion The application of breast plastic surgery combined with breast conserving radical surgery for patients with breast cancer can improve the surgery-related index,promote postoperative rehabilitation,improve the breast aesthetic effect,reduce the incidence of complications and alleviate the trauma reaction.
