AFP、GPC3、GP73、CA125检测对原发性肝癌的诊断价值研究

摘　　要：目的 探讨血清标志物甲胎蛋白(AFP)、磷脂酰肌醇蛋白聚糖3(GPC3)、高尔基体跨膜糖蛋白73(GP73)、糖类抗原125(CA125)单独及联合检测在原发性肝癌诊断中的临床价值。方法 选取原发性肝癌患者80例作为A组,同期良性肝病患者80例作为B组,来院健康体检居民80例作为C组。检测三组血清AFP、GPC3、GP73、CA125水平。对比三组血清AFP、GPC3、GP73、CA125水平,AFP、GPC3、GP73、CA125单独及联合检测对原发性肝癌的诊断效能。结果 A组AFP(398.32±91.62)μg/L、GPC3(71.29±14.25)μg/L、GP73(125.34±1.51)ng/ml、CA125(45.12±2.15)U/ml高于B组的(52.45±11.64)μg/L、(25.32±5.68)μg/L、(15.87±0.29)ng/ml、(38.15±1.15)U/ml和C组的(1.22±0.11)μg/L、(14.32±20.74)μg/L、(3.16±0.64)ng/ml、(5.15±0.64)U/ml,B组高于C组,差异有统计学意义(P<0.05)。联合检测的特异度、准确度均高于AFP、GPC3、GP73、CA125单独检测(P<0.05);联合检测的敏感度与AFP、GPC3、GP73、CA125单独检测对比无差异(P>0.05)。结论 AFP、GPC3、GP73、CA125四项指标联合检测对原发性肝癌有较高的诊断价值,能提高诊断效能,适合推广应用。Objective To explore the clinical value of single and combined detection of alphafetoprotein(AFP),glypican-3(GPC3),golgi protein-73(GP73) and carbohydrate antigen 125(CA125) for the diagnosis of primary liver cancer.Methods 80 patients with primary liver cancer were selected as group A,80 patients with benign liver disease were selected as group B,and 80 healthy residents who came to the hospital for physical examination were selected as group C.The serum levels of AFP,GPC 3,GP73,and CA125 tested in the three groups.The serum levels of AFP,GPC3,GP73 and CA125 of the three groups were compared,as well as the diagnostic efficacy of AFP,GPC3,GP73 and CA125 alone and in combination.Results Group A had AFP of(398.32±91.62) μg/L,GPC3 of(71.29±14.25) μg/L,GP73 of(125.34±1.51) ng/ml,CA125 of(45.12±2.15) U/ml,GPC3 of(71.29±14.25) μg/L,GP73 of(125.34±1.51) ng/ml and CA125 of(45.12±2.15) U/ml,which were higher than that of group B [(52.45±11.64) μg/L,(25.32±5.68) μg/L,(15.87±0.29) ng/ml and(38.15±1.15) U/ml],and group C [(1.22±0.11) μg/L,(14.32±20.74) μg/L,(3.16±0.64) ng/ml and(5.15±0.64) U/ml];group B was higher than group C;the difference was statistically significant(P<0.05).The specificity and accuracy of combined detection were higher than those of AFP,GPC3,GP73 and CA125alone(P<0.05).There was no difference in sensitivity between combined detection and single detection of AFP,GPC3,GP73 and CA125(P>0.05).Conclusion The combined detection of AFP,GPC 3,GP73 and CA125 has high value for the diagnosis of primary liver cancer,and can improve the diagnostic efficiency and is suitable for promotion and application.
