摘　　要：目的探究游离股前外侧皮瓣(ALTFF)用于重建口腔颌面恶性肿瘤切除术后缺损的临床效果及语言功能、吞咽功能的影响。方法回顾性分析2021年6月至2024年4月联勤保障部队第九九〇医院收治的63例行口腔颌面恶性肿瘤切除术的患者,根据取皮瓣部位的不同分为游离上臂外侧皮瓣(LAFF)组(30例)和ALTFF组(33例)。比较两组患者的皮瓣生存率和手术相关指标、语言功能、吞咽功能(SSA)和并发症。结果LAFF组出现1例皮瓣完全坏死,成活率为96.67%,ALTFF组中皮瓣离体移植后均未见皮瓣坏死,成活率为100%,两组比较,差异无统计学意义(P>0.05);两组患者手术时间、皮瓣制备时间、术中出血量、住院时间比较,差异无统计学意义(P>0.05);术后6个月,两组患者的舌根音语音清晰度(PI)值、舌尖音PI值和边音PI值以及SSA评分均升高,且ALTFF组高于LAFF组,差异有统计学意义(P<0.05);ALTFF组患者并发症发生率为3.03%,低于LAFF组的20.00%,差异有统计学意义(P<0.05)。结论ALTFF用于重建口腔颌面恶性肿瘤切除术后缺损临床效果显著,皮瓣生存率高,同时还能促进患者语言功能和吞咽功能的恢复,且并发症发生率低,临床应用价值较高。Objective To explore the clinical effect of(ALTFF flap)for reconstruct the defect of oral and maxillofacial malignancy and the effects of language and swallowing function.Methods The clinical data of 63 patients undergoing oral and maxillofacial malignant resection in our hospital from June 2021 to April 2024 were retrospectively analyzed according to the lateral upper arm flap(LAFF)(n=30)and ALTFF(n=33).Flap survival and surgery-related measures,language function,swallowing function(SSA)and complications were compared between the two groups.Results Statistics show that,One complete flap necrosis occurred in the LAFF group,Survival rate was 96.67%,No flap necrosis was observed after isolated transplantation in the ALTFF group,The survival rate was 100%,However,there was no significant difference between the two groups(P>0.05);There was no significant difference between the operation time,flap preparation time,intraoperative blood loss,and hospital stay time between the two groups(P>0.05);Six months after the surgery,In both groups,tongue root voice clarity(PI),tongue tip PI,and edge PI,and SSA score were increased,And the ALTFF group was higher than the LAFF group(P<0.05);The complication rate of patients in the ALTFF group was 3.03%,Lower than 20.00%in the LAFF group(P<0.05).Conclusions ALTFF is effective for the reconstruction of oral and maxillofacial malignant tumors,with high flap survival rate,and promotes the recovery of language function and swallowing function,with low complication rate and high clinical application value.
