带蒂背阔肌肌瓣乳房修复术与传统背阔肌乳房修复术在早期乳腺癌保乳手术后的应用比较

摘　　要：目的比较带蒂背阔肌肌瓣乳房修复术与传统背阔肌乳房修复术在早期乳腺癌保乳手术后的临床效果。方法选取2022年1月至2024年6月永城市人民医院收治的80例早期乳腺癌患者,按治疗方法不同分为研究组患者40例和对照组40例,研究组患者采用带蒂背阔肌肌瓣乳房修复术,对照组患者采用传统背阔肌乳房修复术,比较两组患者手术情况、乳房美容效果优良率、术后并发症及患者满意度。结果研究组患者手术时间(156.75±13.71)min、住院时间(10.09±1.47)d短于对照组的(178.55±14.56)min、(11.85±1.63)d,术中出血量(233.81±16.59)mL,少于对照组的(260.96±18.49)mL,差异有统计学意义(P<0.05);研究组患者的乳房美容效果优于对照组,术后并发症发生率低于对照组,患者满意度高于对照组,上述差异均有统计学意义(P<0.05)。结论带蒂背阔肌肌瓣乳房修复术相比传统背阔肌乳房修复术具有显著优势,值得临床推广应用。Objective To compare the application of latissimus dorsi muscle pedicled flap breast repair with traditional latissimus dorsi muscle breast repair after breast conserving surgery for early breast cancer.Methods Eighty patients with early breast cancer admitted to Yongcheng People's Hospital from January 2022 to June 2024 were selected and randomly divided into a study group of 40 cases and a control group of 40 cases.The study group used pedicled latissimus dorsi muscle flap breast repair,and the control group used traditional latissimus dorsi muscle breast repair.The two groups were compared in terms of the operation,the excellent rate of breast beauty,postoperative complications and patient satisfaction.Results The surgical time(156.75±13.71)minutes and hospitalization time(10.09±1.47)days of the study group patients were shorter than those of the control group(178.55±14.56)minutes and(11.85±1.63)days.The intraoperative blood loss was(233.81±16.59)mL,which was less than that of the control group(260.96±18.49)mL,and the difference was statistically significant(P<0.05);The breast cosmetic effect of the research group patients was better than that of the control group,the incidence of postoperative complications was lower than that of the control group,and the patient satisfaction was higher than that of the control group.The above differences were statistically significant(P<0.05).Conclusions The pedicled latissimus dorsi muscle flap breast repair surgery has significant advantages over traditional latissimus dorsi muscle breast repair surgery and is worthy of clinical promotion and application.
