EB病毒编码mRNA原位杂交和免疫组织化学CD35 CD21三染法在炎性滤泡树突细胞肉瘤中的应用

摘　　要：目的分享在诊断EB病毒阳性炎性滤泡树突状细胞肉瘤(IFDCS)中免疫组化三色染色技术的经验。方法选用2例肝和脾EBV阳性IFDCS患者(年龄分别为24岁和68岁的女性,肿瘤直径在30~55 mm)进行CD21和CD35的免疫组化单染和EBV编码mRNA(EBER)原位杂交及CD35、CD21、EBER三染,并回顾了相关文献。结果在病理切片显微镜下,可见梭形或卵圆形肿瘤细胞呈席文状或束状排列。这些非典型细胞的细胞核显示为囊泡状染色质和小而明显的核仁。免疫组化和原位杂交结果显示,肿瘤细胞CD21、CD35均呈阳性。EBER原位杂交在2例病例中也有阳性结果。光学显微镜下观察,免疫组化CD21、CD35单色染色肿瘤细胞细胞质呈棕黄色阳性,EBER原位杂交肿瘤细胞呈细胞核棕黄色阳性。EBER-CD35-CD21三染:肿瘤细胞EBER显示细胞核呈棕黄色阳性,CD35显示细胞质呈棕黄色阳性,CD21显示细胞质呈红色阳性。结论在同一切片上对组织进行CD21、CD35和EBER染色能同时显示基因和蛋白水平2个层面的3种结果,能较为直观地分析IFDCS肿瘤细胞。Objective To share our insights on the immunohistochemical trichrome staining technique utilized in the diagnosis of Epstein-Barr virus-positive follicular dendritic cell sarcoma(FDCS).Methods In this study,two patients with EBV-positive inflammatory follicular dendritic cell sarcoma(IFDCS)involving the liver and spleen,aged 24 and 68 years old respectively,and with tumor diameters ranging from 30 to 55 mm,were selected for immunohistochemical staining of CD21 and CD35,in situ hybridization for EBV encoded mRNA(EBER),and triple staining for CD35,CD21,and EBER,and reviewed the pertinent literature.Results Under the microscope of pathological section,the fusiform or oval tumor cells were arranged in schiff-like or fascicular arrangement.The nuclei of these atypical cells show vesicular chromatin and small,distinct nucleolus.The results of immunohistochemistry and in situ hybridization showed that CD21 and CD35 were positive.EBER in situ hybridization also showed positive results in 2 cases.The tumor cells were spindle-shaped or oval with a zonal or fascicular arrangement.These atypical cells exhibited vesicular chromatin and small,conspicuous nucleoli.Immu-nohistochemistry and in situ hybridization revealed that the tumor cells were positive for CD21 and CD35,respectively.EBER in situ hybridization was also positive in both cases.Conclusion Staining for CD21,CD35,and EBER on the same slide allows for the simultaneous visualization of three outcomes at both the gene and protein levels,enabling a more intuitive analysis of IFDCS tumor cells.
