根尖囊肿微创术联合根管倒填充术治疗根尖周病损对骨吸收的影响

摘　　要：目的:研究根尖囊肿微创术联合根管倒填充术对根尖周病损的改善作用及对病损周骨吸收的影响。方法:选择2020年4月-2022年10月笔者医院就诊的71例根尖周病损患者进行本次研究,按照随机数表法分为两组,对照组采用根管填充治疗,研究组采用根尖囊肿微创术联合根管倒填充术治疗。对比两组临床疗效、牙周状况指标、根尖病变评分及根尖周骨吸收恢复情况。结果:研究组总有效率显著高于对照组(86.11%vs.62.86%)(P<0.05);研究组检测菌斑指数(Plaque index,PLI)、龈沟出血指数(Sulcus bleeding index,BI)、探诊深度(Probing depth,PD)、临床附着丧失(Clinical attachment loss,CAL)均显著低于对照组(P<0.05);研究组治疗后3个月,根尖病变评分显著低于对照组[(1.13±0.10)分vs.(1.51±0.17)分](P<0.05);研究组治疗后12个月愈合率显著高于对照组(83.33%vs.60.00%)(P<0.05)。结论:根尖囊肿微创术联合根管倒填充术对根尖周病损疾病的临床症状改善效果显著,可有效降低根尖周骨吸收。Objective To study the effect of minimally invasive surgery combined with root canal filling surgery on bone resorption in periapical lesions.Methods 71 in patients with periapical who received treatment in the hospital from April 2020 to October 2022 were selected for this study,according to random number table method,they were divided into two groups,the control group received root canal filling treatment,the research group used minimally invasive surgery for apical cysts combined with root canal filling surgery for treatment.Compare the clinical efficacy,periodontal status indicators,apical lesion scores,and periapical bone resorption recovery between two groups.Results The total effective rate of the study group was significantly higher than that of the control group(86.11%vs.62.86%)(P<0.05).The PLI,BI,PD and CAL of the study group were significantly lower than those of the control group(P<0.05).After 3 months of treatment,the apical lesion score in the research group was significantly lower than that in the control group[(1.13±0.10)scores vs.(1.51±0.17)scores](P<0.05).The healing rate of the study group at 12 months after surgery was significantly higher than that of the control group(83.33%vs.60.00%)(P<0.05).Conclusion The combination of minimally invasive surgery for apical cysts and root canal filling surgery has a significant improvement effect on clinical symptoms of periapical lesions and can effectively reduce periapical bone resorption.
