摘　　要：目的分析种植体周炎(peri-implantitis,PI)患者种植体周黏膜沟液基质金属蛋白酶-7(matrix metalloproteinase-7,MMP-7)和MMP-8水平,分析其临床及预后评估价值。方法选取郑州大学第一附属医院口腔科收治的72例PI患者为PI组,同期另选50例种植体周健康者为对照组。检测比较2组种植体周黏膜沟液MMP-7及MMP-8水平,并分析其与PI患者牙周临床指标的相关性。根据急性生理学与慢性健康状况评分Ⅱ(acute physiology and chronic health evaluationⅡ,APACHEⅡ)将PI患者分为预后良好组(n=49)与预后不良组(n=23),比较2组种植体周黏膜沟液MMP-7及MMP-8水平,并采用受试者工作特征(the receiver operating characteristic,ROC)曲线分析软件,分析其对不良预后的预测效能。结果PI组改良菌斑指数(modified plaque index,mPLI)、改良龈沟出血指数(modified sulcus bleeding index,mSBI)、探诊深度(probing depth,PD)及种植体周黏膜沟液MMP-7及MMP-8水平均明显高于对照组(均P<0.05),相关性分析显示,种植体周黏膜沟液MMP-7及MMP-8水平与mPLI、mSBI及PD均呈正相关(r>0,P<0.05);预后不良组种植体周黏膜沟液MMP-7及MMP-8水平均高于预后良好组(均P<0.05);ROC曲线分析发现,种植体周黏膜沟液MMP-7及MMP-8水平预测PI患者不良预后的曲线下面积(AUC)分别为0.712和0.786,二者联合预测不良预后的AUC值为0.823,明显高于MMP-7的预测价值(Z=2.067,P=0.039)。结论种植体周黏膜沟液MMP-7及MMP-8水平与PI患者种植体周临床指标密切相关,且对不良预后具有一定的预测价值。Objective To analyze the levels of matrix metalloproteinase-7(MMP-7)and MMP-8 in peri-implant mucosal sulcus fluid and their correlation with periodontal clinical indexes,as well as to evaluate their prognostic significance in patients with peri-implantitis(PI).Methods 72 patients with PI in our hospital were selected as the PI group,and another 50 peri-implant healthy individuals were selected as the control group.The levels of MMP-7 and MMP-8 in the peri-implant mucosal sulcus fluid were detected and compared between the two groups.Their correlation with periodontal clinical indexes in PI patients was analyzed.Then,PI patients were divided into a good prognosis group(n=49)and a poor prognosis groups(n=23)according to the acute physiology and chronic health evaluationⅡ(APACHEⅡ)score.Peri-implant mucosal sulcus fluid MMP-7 and MMP-8 levels were compared between the two subgroups,and the receiver operating characteristic(ROC)curve was used to analyze their predictive efficacy of these markers for poor prognosis in PI patients.Results The modified plaque index(mPLI),modified sulcus bleeding index(mSBI),probing depth(PD),and levels of MMP-7 and MMP-8 in the peri-implant mucosal sulcus fluid in the PI group were significantly higher than the control group(all P<0.05).Correlation analysis showed that peri-implant mucosal sulcus fluid MMP-7 and MMP-8 levels were positively correlated with mPLI,mSBI,and PD(r>0,P<0.05).The peri-implant mucosal sulcus fluid MMP-7 and MMP-8 levels in the poor prognosis group were higher than those in the good prognosis group(all P<0.05).ROC curve analysis showed that the area under the curve(AUC)for the peri-implant mucosal sulcus fluid MMP-7 and MMP-8 levels in predicting poor prognosis in PI patients was 0.712 and 0.786,respectively.The AUC for the combination of MMP-7 and MMP-8 in predicting poor prognosis was 0.823,which was significantly higher than that for MMP-7 alone(Z=2.067,P=0.039).Conclusion MMP-7 and MMP-8 levels in the peri-implant mucosal sulcus fluid are closely related
