摘　　要：肝细胞癌(hepatocellular carcinoma,HCC)是我国最常见的恶性肿瘤,肝细胞癌合并门静脉癌栓(portal vein tumor thrombus,PVTT)发生率高,预后差,严重威胁患者的生命健康。2021年版中国医师协会肝癌专委会指南指出手术切除是PVTTⅠ、Ⅱ型患者首选并有可能获得根治机会的治疗方法。对于初次发现就已经失去手术机会的患者,转化治疗为其提供了后续接受手术治疗的可能。指南推荐转化治疗的方式主要是以放疗或以免疫及靶向治疗为主,多种治疗方式联合,使不可切除肿瘤转化为可切除的肿瘤,极大地提高了该类患者的生存率。该文主要概括了肝癌合并门静脉癌栓转化治疗的临床研究进展。Hepatocellular carcinoma(HCC)is the most common malignant tumor in China,with a high incidence of portal vein cancer embolus(PVTT)and poor prognosis,which seriously threatens the life and health of patients.The 2021 edition of the Chinese Medical Doctor Association Liver Cancer Committee Guidelines pointed out that surgical resection is the first choice for PVTT typeⅠandⅡpatients and may provide a chance for radical treatment.Conversion therapy offers the possibility of subsequent surgery for other patients who have already lost the opportunity for surgery when first identified.The guidelines recommend that conversion therapy is mainly based on radiotherapy or immune and targeted therapy,and the combination of multiple treatment methods can transform unresectable tumors into resectable tumors,which greatly improves the survival rate of such patients.This article mainly summarizes the clinical research progress of transformation therapy of hepatocellular carcinoma complicated with portal vein tumor thrombus.
