基于复杂网络分析吕文良教授治疗慢性萎缩性胃炎遣方用药规律研究

摘　　要：目的 探讨吕文良教授治疗慢性萎缩性胃炎的辨证思路及用药特色。方法 收集2017年1月至2023年5月吕教授在中国中医科学院广安门医院脾胃病门诊治疗慢性萎缩性胃炎的完整医案。采用Microsoft Excel 2021建立规范数据库,进行药物频次、四气、五味、归经、功效类别统计。采用SPSS Statistics 26.0、Gaphi0.10.1对高频药物进行复杂网络分析,包括中药间关联规则分析、因子分析及模块化聚类分析,探讨核心药物聚类方。结果 共纳入处方515首,涉及中药221味,使用频次排前5位的中药依次为山楂、黄芪、黄芩、茯苓、麦芽;药性主要为温、寒、平;药味以苦、甘为主;归经以脾、肺、肝、胃经为主;药物功效主要为补虚、清热、理气。对用药频次前32位处方药物进行复杂网络分析共产生28条关联,9个公因子,2个模块聚类,3个聚类方。结论 吕教授治疗慢性萎缩性胃炎以益气健脾、清热活血为主要治法,尤重调理脏腑功能,处方药物性味和合,守衡为要,为临床治疗提供指导意义。Objective To explore syndrome differentiation thinking and medication characteristics of Professor LüWenliang in the treatment of chronic atrophic gastritis.Methods Complete medical records of Professor LüWenliang in the treatment of chronic atrophic gastritis at Spleen and Stomach Disease Outpatient Department of Guang’anmen Hospital in China Academy of Chinese Medical Sciences from January 2017 to May 2023 were collected.Standardized database was established by using Microsoft Excel 2021,the frequency,four qi,five flavors,channel tropism,and efficacy categories of drugs were statistically analyzed.SPSS Statistics 26.0 and Gaphi0.10.1 were used to conduct complex network analysis on high-frequency drugs,included association rule analysis,factor analysis,and modular clustering analysis among Chinese materia medica.Clustering formulas of core drugs were explored.Results A total of 515 prescriptions were included,involved 221 Chinese materia medica.The top five Chinese materia medica with the highest frequency of use were Crataegi Fructus,Astragali Radix,Scutellariae Radix,Poria,Hordei Fructus Germinatus;medicinal natures were warm,cold,and flat;medicinal flavors were mainly bitter and sweet;main channel tropisms were spleen,lung,liver,and stomach channels;the main efficacy of drugs were to tonify deficiency,clear heat,and regulate qi.A complex network analysis was conducted on the top 32 prescription drugs in terms of medication frequency,resulted in 28 associations,9 common factors,2 module clusters,and 3 clustering methods.Conclusion Professor LüWenliang in the treatment of chronic atrophic gastritis mainly focuses on tonifying qi and strengthening spleen,clearing heat and promoting blood circulation,with a particular emphasis on regulating function of zang fu viscera.Prescription drugs should have a harmonious nature and flavor,and guarding the balance is essential,providing guidance for clinical treatment.
