摘　　要：目的 深入了解产科医护人员照护妊娠合并抗磷脂综合征患者的真实体验,为临床优化照护及管理策略提供参考。方法 采用现象学研究法,通过目的抽样法在2023年11月至2024年5月选取内蒙古医科大学附属医院、内蒙古自治区人民医院、内蒙古妇幼保健院的17名产科医护人员,进行半结构式访谈,采用Colaizzi 7步分析法分析资料并提炼主题。结果 从访谈结果中提炼出5个主题:专业素养挑战,包括疾病复杂性认知、诊疗方案精细权衡及知识更新的紧迫性;心理情感重压,包括深度共情患者及自我心理调适挣扎;团队协作磨合,包括初期协作困境及协作优化转机;复杂的职业体验,包括使命驱动责任感及无力感常伴左右;照护中探寻患者需求,包括心理重建需关注及需要持续性专业守护。结论 产科医护人员在照护妊娠合并抗磷脂综合征患者中的多维度体验,对改善临床照护实践、加强医护团队建设和提升患者医疗护理质量具有指导意义。Objective To gain an in-depth understanding of the real experiences of obstetric medical staff in caring for pregnancy patients complicated with antiphospholipid syndrome,and to provide a reference for optimizing clinical care and management strategies.Methods A phenomenological research method was adopted.Through purposive sampling,17 obstetric medical staff from the Affiliated Hospital of Inner Mongolia Medical University,Inner Mongolia Autonomous Region People’s Hospital,and Inner Mongolia Maternity and Child Health Care Hospital were selected from November 2023 to May 2024.Semi-structured interviews were conducted,and Colaizzi 7-step analysis method was used to analyze the data and extract themes.Results Five themes were extracted from the interview results:challenges to professional competence,including the understanding of the complexity of the disease,the delicate balance of diagnosis and treatment plans,and the urgency of knowledge updating;heavy psychological and emotional pressure,including deep empathy for patients and the struggle with self-psychological adjustment;the running-in of team collaboration,including initial collaboration difficulties and the turning point of collaborative optimization;complex professional experiences,including a sense of responsibility driven by the mission and the frequent presence of a sense of powerlessness;exploring the needs of patients during care,including the need to pay attention to psychological reconstruction and the need for continuous professional protection.Conclusion This study reveals the multi-dimensional experiences of obstetric medical staff in caring for pregnancy patients complicated with antiphospholipid syndrome,which has guiding significance for improving clinical care practices,strengthening the construction of the medical team,and enhancing the quality of medical care for patients.
