摘 要:目的探讨牙周基础治疗前后龈沟液miRNA-23a、miRNA-146a表达变化及临床意义。方法选取2019年10月至2022年4月郑州颐和医院收治的170例牙周炎患者以及同期68例健康体检者作为研究对象,分别作为研究组和对照组;并根据研究组患者牙周炎程度分为轻度患者65例、中度患者58例和重度患者47例。采用酶联免疫吸附试验法检测并比较治疗前和治疗6周后龈沟液中的白细胞介素(IL)-1β、IL-6、IL-10、IL-4、肿瘤坏死因子α(TNF-α)、γ干扰素(INF-γ)表达水平;实时荧光定量PCR法检测并比较龈沟液miRNA-23a、miRNA-146a表达水平,同时分析miRNA-23a、miRNA-146a表达水平与牙周炎患者各炎症因子及临床指标之间的关系。结果研究组龈沟液中miRNA-23a、miRNA-146a以及促炎性细胞因子(IL-1β、IL-6、TNF-α)表达水平均高于对照组(P<0.05),研究组龈沟液中抗炎细胞因子IL-10、IL-4/INF-γ表达水平均低于对照组(P<0.05),且轻、中、重度3组上述因子差异有统计学意义(P<0.05)。治疗后3组患者miRNA-23a、miRNA-146a以及临床指标[附着丧失(AL)、牙周探诊深度(PD)、探诊出血(BOP)阳性率]相比治疗前均下降(P<0.05)。Pearson相关性分析结果显示龈沟液中miRNA-23a、miRNA-146a表达水平与炎症因子IL-1β、IL-6、TNF-α表达水平以及AL、PD呈正相关,与IL-10表达水平呈负相关(P<0.05)。结论牙周炎患者龈沟液miRNA-23a、miRNA-146a存在显著高表达,且与患者牙周炎炎症因子表达水平存在显著相关性;牙周基础治疗可以通过降低miR-23a、miRNA-146a,改善牙周炎患者炎症反应程度。Objective To explore the clinical significance of changes in expression of miRNA-23a and miRNA-146a in gingival crevicular fluid before and after periodontal treatment.Methods From October 2019 to April 2022,170 patients with periodontitis treated in Zhengzhou Yihe Hospital and 68 healthy medical examiners during the same period were selected as the research subjects,and were used as the study group and the control group,respectively.According to the degree of periodontitis in the study group,the patients were divided into mild,moderate and severe patients,65,58 and 47 patients.The expression levels of interleukin(IL)-1β,IL-6,IL-10,tumor necrosis factor-α(TNF-α),IL-4 and interferon-γ(INF-γ)in gingival crevicular fluid were detected by the enzyme-linked immunosorbent assay before and after treatment for 6 weeks.The expression levels of miRNA-23a and miRNA-146a in the gingival crevicular fluid were detected by real-time fluorescence quantitative PCR before and after treatment for 6 weeks.The relationship between the expression levels of miRNA-23a and miRNA-146a and the inflammatory factors and clinical indicators of patients with periodontitis were also analyzed.Results The expression levels of miRNA-23a,miRNA-146a and proinflammatory cytokine(IL-1β,IL-6,TNF-α)in the gingival crevicular fluid of the study group were higher than those of the control group(P<0.001).The expression levels of anti-inflammatory cytokine IL-10 and IL-4/INF-γof the study group were lower than those of the control group(P<0.05),and there was a statistically significant difference between the above factors in the three groups of mild,moderate and severe(P<0.05).After treatment,miRNA-23a,miRNA-146a,and clinical indicators(AL,PD,BOP positive rate)all decreased significantly compared with those before treatment(P<0.05).Pearson correlation analysis showed that the expression levels of miRNA-23a and miRNA-146a in gingival crevicular fluid were significantly positively correlated with the expression levels of inflammatory factors IL-1β,IL
