摘  要:目的观察早期免疫强化型肠内营养联合活菌制剂对营养不良的溃疡性结肠炎(UC)患者机体免疫、肠黏膜屏障的影响。方法选取2022年1月至2024年1月濮阳市人民医院收治的UC患者87例,根据治疗方案分为两组,其中接受普通肠内营养联合活菌制剂治疗的43例为对照组,接受免疫强化型肠内营养联合活菌制剂治疗的44例为研究组。对比两组疗效、营养状况[血红蛋白(Hb)、前白蛋白(PA)、白蛋白(ALB)]、肠黏膜屏障功能[二胺氧化酶(DAO)、D-乳酸(D-LA)、内毒素(ET)]、免疫功能[补体C3、免疫球蛋白(Ig)A、IgG]及不良反应。结果研究组总有效率(93.18%)高于对照组(76.74%)(P<0.05);治疗14 d后研究组Hb、ALB、PA高于对照组(P<0.05);治疗14 d后研究组DAO、ET、D-LA低于对照组(P<0.05);治疗14 d后研究组IgA、IgG高于对照组,补体C3低于对照组(P<0.05);两组不良反应发生率(2.27%比9.30%)对比,差异无统计学意义(P>0.05)。结论免疫增强型肠内营养联合活菌制剂治疗营养不良的UC疗效显著,可改善营养状况、肠黏膜屏障功能、免疫功能,安全性较高。Objective To investigate the effect of live bacteria preparation combined with early immune-enhacing enteral nutrition on the body immunity and intestinal mucosal barrier in patients with malnutrition ulcerative colitis(UC).Methods A total of 87 UC patients in Puyang People’s Hospital from January 2022 to January 2024 were selected and divided them into two groups according to different treatment plans.Among them,43 patients who received regular enteral nutrition combined with live bacteria preparations were in the control group,and 44 patients who received immune-enhancing enteral nutrition combined with live bacteria preparations were in the study group.The curative effect,nutritional status[hemoglobin(Hb),prealbumin(PA),albumin(ALB)],intestinal mucosal barrier function[diamine oxidase(DAO),D-lactic acid(D-LA),endotoxin(ET)],immune function[complement C3,immunoglobulin A(IgA),IgG]and adverse reactions were compared between the two groups.Results The total effective rate of the study group(93.18%)was higher than that of the control group(76.74%)(P<0.05).After 14 days of treatment,Hb,ALB and PA in the study group were higher than those in the control group(P<0.05).After 14 days of treatment,DAO,ET and D-LA in the study group were lower than those in the control group(P<0.05).After 14 days of treatment,IgA and IgG in the study group were higher than those in the control group,while complement C3 was lower than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(2.27%vs 9.30%)(P>0.05).Conclusion The treatment of malnutrition in UC with immune-enhancing enteral nutrition combined with live bacterial preparations has significant therapeutic effects,improving nutritional status,intestinal mucosal barrier function,and immune function,with high safety.
