摘 要:目的探讨间苯二酚碱性品红弹力纤维与细胞角蛋白7(CK7)免疫组织化学的双重染色法在明确诊断肺腺癌胸膜侵犯分级中的作用。方法选取2024年1月至6月期间由中山大学孙逸仙纪念医院病理科明确诊断的肺腺癌组织标本30例,分别行苏木精-伊红(HE)染色、弹力纤维染色及免疫组织化学和弹力纤维双重染色,观察这三种染色方法在能否明确诊断肺腺癌胸膜侵犯分级的差别。结果单由常规HE染色及弹力纤维染色明确诊断肺腺癌胸膜侵犯分级的均为3例,占10%,而间苯二酚碱性品红与CK7双重染色将肿瘤细胞染成棕黄色,弹力纤维染成蓝黑色,细胞核染成红色,可以将30例标本100%的明确诊断肺腺癌胸膜侵犯分级,3组染色比较差异有统计学意义(χ^(2)=49.091,P<0.001)。结论间苯二酚碱性品红与CK7双重染色法将肿瘤细胞与脏层胸膜弹力层的关系直观清晰地显示出来,能很好地判断肿瘤细胞是否侵犯胸膜,有利于胸膜侵犯分段的评估,为肺腺癌双重染色技术提供重要补充。Objective To investigate the role of dual staining using resorcinol basic fuchsin for elastic fibers and cytokeratin 7(CK7)immunohistochemistry in clarifying the grading of pleural invasion in lung adenocarcinoma.Methods Thirty lung adenocarcinoma tissue specimens diagnosed by the Department of Pathology at Sun Yat-sen Memorial Hospital from January 2024 to June 2024 were selected.Hematoxylin-eosin(HE)staining,elastic fiber staining,and dual staining combining immunohistochemistry(CK7)with elastic fiber staining were performed.The differences among these three methods in determining pleural invasion grading were analyzed.Results Conventional HE staining and elastic fiber staining alone definitively diagnosed pleural invasion grading in 3 cases(10%).In contrast,the dual staining method(resorcinol basic fuchsin and CK7)stained tumor cells brown-yellow,elastic fibers blue-black,and nuclei red,enabling 100%definitive diagnosis of pleural invasion grading in all 30 cases.Statistical analysis revealed significant differences among the three staining methods(χ^(2)=49.091,P<0.001).Conclusion The dual staining method with resorcinol basic fuchsin and CK7 visually and clearly demonstrates the relationship between tumor cells and the elastic layer of the visceral pleura.It effectively identifies tumor cell invasion into the pleura and facilitates the assessment of pleural invasion grading,providing an important complementary technique for dual staining in lung adenocarcinoma.
