居家医疗参与者满意度影响因素关联分析

摘  要:目的:从服务需求方老年患者群体和提供方医护人员的角度出发,梳理两类参与者的个人特征及居家医疗满意度影响因素特征,挖掘不同类型居家医疗参与者的满意度影响因素的关联规则。方法:对具有居家医疗经历的患者和医护人员进行问卷调查,并采用Apriori算法对问卷调查数据进行关联规则分析。结果:对病患而言,慢病患者重视与上门医护人员的熟悉度;非慢病患者或对服务质量要求高的患者更偏好高级别医疗机构提供的服务;绝大多数患者认为居家医疗法律的规范化重要程度很高,并且希望能获得高质量的医疗服务等;对上门医护人员而言,服务时间段重要性与出行方式重要性呈正相关,且工作年限较久的或高级别医疗机构的医护人员认为上门服务的时间范围对居家医疗满意度的影响较大。结论:医疗机构提供个性化的居家医疗服务,加强相关人员培训,保证居家医疗服务质量;医疗机构引入排班管理系统,合理安排上门医护人员的服务时间段和出行方式;政府部门应考虑向低收入人群提供居家医疗补贴,以减轻低收入老年群体的医疗负担;应出台扶持政策,激励高级别医院医护人员投身居家医疗服务;不断完善相关法律法规,确保居家医疗服务的安全与质量。Objective From the demand-side of elderly group and the supply-side of HHC workers,to sort out the participants'characteristics and the characteristics of the factors influencing satisfaction,and explore the association rules of influencing factors of satisfaction of different types of HHC participants.Methods A questionnaire survey was conducted to patients and healthcare professionals with experience of home healthcare.Data were analyzed using the Apriori algorithm for association rules.Results For HHC patients,chronic disease patients valued the familiarity of the HHC workers,while non-chronic patients or patients with higher demands on service attitudes preferred services provided by first-class hospital.The vast majority of patients thought that the standardization of HHC laws was of great importance,and wanted to have access to high-quality HHC services.For HHC workers,the importance of service timeframe was positively correlated with the importance of travelling mode.HHC workers who have worked for a long time or work in high-level medical institutions believed that the timeframe of HHC service had a greater impact on the satisfaction of HHC.Conclusion Medical institutions should provide personalized HHC services,strengthen the training of HHC workers,ensure the quality of HHC services,introduce a scheduling management system to reasonably arrange the service timeframe and travel mode of HHC workers.Government departments should consider providing low-income people with subsidies for home health care to reduce the medical burden of low-income elderly groups,formulate supportive policies to motivate high-level hospital staff to devote themselves to HHC services,and continuously improve relevant laws and regulation to ensure the safety and quality of HHC services.
