摘 要:目的探讨不同清肠药物对肠道准备情况及肠道群菌的影响。方法选取2021年1月至2024年6月于新乡医学院第一附属医院行肠镜检查的183例患者为研究对象,采用随机数表法分为A、B、C三组,每组61例,A组服用聚乙二醇电解质进行肠道准备,B组服用硫酸镁进行肠道准备,C组服用番泻叶进行肠道准备,比较三组首次排便时间、肠道准备时间、进镜时间、肠道清洁度、药物耐受性、息肉检出情况及肠道群菌。结果三组肠道准备时间差异有统计学意义(P<0.05),且A组<B组<C组(P<0.05)。三组进镜时间差异有统计学意义(P<0.05),且A组和B组明显短于C组(P<0.05)。三组首次排便时间差异无统计学意义(P>0.05)。三组肠道清洁度差异有统计学意义(P<0.05),且A组和B组左侧结肠、中间结肠、右侧结肠及总分明显高于C组(P<0.05),A组与B组间各评分差异无统计学意义(P>0.05)。3组药物耐受性差异有统计学意义(P<0.05),且A组和B组药物耐受性明显高于C组(P<0.05),A组与B组间差异无统计学意义(P>0.05)。A组息肉检出率及扁平息肉检出率明显高于C组(P<0.05),A组与B组间、B组与C组间息肉检出情况差异无统计学意义(P>0.05)。检查结束后第1天三组细菌总数和革兰阳性菌数量均明显低于服药前(P<0.05),肠球菌和大肠杆菌数量明显高于服药前(P<0.05),但A组细菌总数和革兰阳性菌数量明显高于同期B组、C组(P<0.05),B组细菌总数和革兰阳性菌数量明显高于同期C组(P<0.05),且A组、B组肠球菌和大肠杆菌数量明显低于同期C组(P<0.05)。结论相对于清肠药物硫酸镁、番泻叶,聚乙二醇电解质在提高肠道准备质量、药物耐受性、息肉检出情况方面更具优势,且对肠道菌群的影响更小,值得推荐。Objective To explore the influence under different intestinal clearing drugs on intestinal preparation status and intestinal flora.Methods Totally 183 patients who received enteroscopy in the First Affliated Hospital of Xinxiang Medical University from January 2021 to June 2024 were selected as the research subjects,and were divided into group A,group B and group C by random number table method,with 61 cases in each group.Group A was given polyethylene glycol electrolyte for intestinal preparation,and group B took magnesium sulfate for intestinal preparation,and group C was treated with folium sennae.The first defecation time,intestinal preparation time,enteroscope entry time,intestinal cleanliness,drug tolerance,polyp detection status and intestinal flora were compared among the three groups.Results There was a statistical significance in intestinal preparation time among the three groups(P<0.05),and the intestinal preparation time was manifested as group A<group B<group C(P<0.05).The enteroscope entry time revealed a statistical difference among the three groups(P<0.05),and the time in group A and group B was significantly shorter than that in group C(P<0.05).The first defecation time was not statistically different among the three groups(P>0.05).There was a statistically significant difference in intestinal cleanliness among the three groups(P<0.05),and the left colon,middle colon,right colon and total score were significantly higher in group A and group B than those in group C(P<0.05),but there were no statistical differences in the above scores between group A and group B(P>0.05).The drug tolerance among the three groups was statistically different(P<0.05),and the drug tolerance in group A and group B was significantly higher than that in group C(P<0.05),but no statistical significance was shown between group A and group B(P>0.05).The detection rates of polyps and flat polyps in group A were significantly higher than those in group C(P<0.05),but there were no statistical differences in the detection rate of
