摘 要:目的:探讨小细胞肺癌患者中神经细胞黏附分子(CD56)、胰岛素瘤相关蛋白1(INSM1)表达及在预后评估中的应用。方法:回顾性选取2021年1月—2023年1月某院确诊的小细胞肺癌患者98例为研究对象,采用免疫组织化学(IHC)技术检测CD56和INSM1的表达;比较不同临床特征患者CD56、INSM1表达情况;采用Kaplan-Meier方法分析CD56、INSM1表达与1年生存率的关系,采用Cox回归分析影响小细胞肺癌患者1年内死亡的危险因素。结果:98例患者中85例CD56阳性表达(86.73%),13例阴性表达(13.27%);82例INSM1阳性表达(83.67%),16例阴性表达(16.33%)。有吸烟史、胸膜侵犯患者的CD56、INSM1阳性表达率高于无吸烟史、无胸膜侵犯患者,差异有统计学意义(P<0.05)。术后1年随访结果显示,存活64例(65.31%),死亡34例(34.69%);CD56阳性表达患者1年生存率(61.18%)低于CD56阴性表达患者(92.31%),INSM1阳性表达患者1年生存率(60.98%)低于INSM1阴性表达患者(87.50%),差异均有统计学意义(P<0.05)。Cox回归分析结果显示,吸烟史、有胸膜侵犯、CD56、INSM1阳性表达为小细胞肺癌患者1年内发生死亡的危险因素(P<0.05)。结论:CD56和INSM1的阳性表达与小细胞肺癌患者的吸烟史、胸膜侵犯密切相关,以上指标是小细胞肺癌患者1年内死亡的独立危险因素。Objective:To explore the expressions of neuronal adhesion molecule(CD56)and insulinoma associated protein 1(INSM1)in patients with small-cell lung cancer and their application in prognosis evaluation.Methods:A retrospective analysis was conducted on 98 small-cell lung cancer patients diagnosed in a hospital from January 2021 to January 2023,and clinical data and pathological features of all the patients were collected.Immunohistochemistry(IHC)technology was used to detect the expressions of CD56 and INSM1.The expressions of CD56 and INSM1 in patients with different clinical characteristics were compared.Kaplan Meier method was used to analyze the relationship between CD56,INSM1 expressions and 1-year survival rate,and Cox regression analysis was used to identify risk factors affecting 1-year mortality in patients with small-cell lung cancer.Results:Among the 98 patients,85 were CD56 positive(86.73%,85/98)and 13 were CD56 negative(13.27%,13/98).There were 82 cases of INSM1 positive expression(83.67%,82/98)and 16 cases of INSM1 negative expression(16.33%,16/98).The positive expression rates of CD56 and INSM1 in patients with smoking history and pleural invasion were higher than those in patients without(P<0.05).The one-year postoperative follow-up of the patients showed that 64 cases(65.31%,64/98)survived and 34 cases(34.69%,34/98)died.The overall survival rate of CD56 positive expression patients was 61.18%(52/85),lower than that of CD56 negative expression patients'92.31%(12/13).The overall survival rate of INSM1 positive expression patients was 60.98%(50/82)lower than that of INSM1 negative expression patients'87.50%(14/16)(P<0.05).Cox regression analysis showed that smoking history,pleural invasion,CD56 positive expression,and INSM1 positive expression were risk factors for death within one year in small-cell lung cancer patients(P<0.05).Conclusion:The positive expressions of CD56 and INSM1 are closely correlated with smoking history and pleural invasion in patients with small-cell lung cancer,and are independe
