摘  要:目的探索基于Gd-EOB-DTPA增强MRI的影像组学在预测肝细胞肝癌(HCC)术后早期复发中的应用价值。方法收集接受根治性切除术且经病理证实为HCC的患者共91名。在动脉期及肝胆期图像上手动分割肿瘤VOI,共提取246个组学特征。使用LAASO回归对组学特征进行降维和筛选,并建立影像组学模型(支持向量机)模型。对临床病理资料进行单因素分析,并分别建立临床病理模型(逻辑回归模型)和综合模型(逻辑回归模型),用于预测HCC术后早期复发(≤1年)。使用ROC曲线和Delong测试评估模型的预测效能和差异。结果我们分别建立了影像组学模型、临床病理模型和综合模型,三个模型的AUC分别是0.879(95%CI:0.782-0.944)、0.861(95%CI:0.760-0.931)和0.881(95%CI:0.850-0.978)。三个模型之间的预测效能无显著性差异。结论基于Gd-EOB-DTPA增强MRI的影像组学模型可以在术前预测HCC患者术后早期复发。Objective To explore the application of Gd-EOB-DTPA enhanced magnetic resonance imaging(MRI)-based radiomics in predicting early recurrence of HCC after curative hepatectomy.Methods A total of 91 patients with hepatocellular carcinoma(HCC)confirmed by pathology were analyzed retrospectively.All of them received curative hepatectomy.A total of 246 radiomic features were extracted from the volumetric of interest(VOI)of the tumor in Gd-EOB-DTPA MR images(based on arterial phase and hepatobiliary phase).The least absolute shrinkage and selection operator method(LASS O)was applied to to reduce the dimension of radiomic features and select the radiomic features.A radiomic model(support vector machine)was developed to predict early recurrence(≤1 year)of HCC after curative hepatectomy.Univariate analysis was used to selected clinicopathological data.The clinicopathological model(logistic regression model)and combined model(logistic regression model)were established respectively to predict early recurrence of HCC after curative hepatectomy.The performance of those models was assessed and compared using receiver operating characteristic(ROC)curve and Delong test.Results We established the radiomic model,clinicopathological model and combined model to predict early recurrence of HCC after curative hepatectomy,with ROC curve(AUC)of 0.879(95%CI:0.782-0.944),0.861(95%CI:0.760-0.931),and 0.881(95%CI:0.850-0.978)respectively.There was no significant difference among them in predicting early recurrence of HCC after curative hepatectomy.Conclusion Radiomic model based on Gd-EOB-DTPA-enhanced MRI can predict early recurrence of HCC after curative hepatectomy.
