血清LRG1、GDF15、GP73对原发性肝癌患者介入治疗后复发的预测价值

摘  要:目的探究原发性肝癌患者血清中富亮氨酸α-2糖蛋白1(LRG1)、生长分化因子15(GDF15)、高尔基体蛋白73(GP73)水平对原发性肝癌患者介入治疗后复发的预测价值。方法选取2019年1月至2022年12月共221例原发性肝癌介入治疗患者为观察组,根据是否复发分组为复发组62例,非复发组159例,选择同期221名体检健康志愿者为对照组。采用酶联免疫(ELISA)法检测两组人员血清中LRG1、GDF15的水平,采用上转发光法检测GP73水平;多因素Logisitc回归分析介入治疗后复发的影响因素;绘制受试者工作特征(ROC)曲线分析血清LRG1、GDF15、GP73水平对原发性肝癌的诊断价值及介入治疗后复发的预测价值。结果与对照组相比,观察组患者血清中LRG1水平显著下降,GDF15和GP73水平显著上升(P<0.05);ROC曲线结果表明血清LRG1、GDF15、GP73水平3者联合诊断原发性肝癌患者曲线下面积(AUC)为0.950,显著大于LRG1(Z=4.572,P<0.001)、GDF15(Z=3.587,P<0.001)和GP73(Z=6.171,P<0.001)单独诊断;与非复发组相比,复发组患者血清中LRG1水平下降,GDF15和GP73水平升高(P<0.05);多因素Logistic回归分析结果显示肝外转移、甲胎蛋白、LRG1、GDF15、GP73是原发性肝癌患者介入治疗后复发的影响因素(P<0.05);ROC结果显示血清中LRG1、GDF15、GP73水平联合预测复发AUC为0.885,显著大于LRG1(Z=3.459,P=0.001)、GDF15(Z=3.367,P=0.001)和GP73(Z=2.341,P=0.019)水平单独预测,具有较高的预测效能。结论原发性肝癌患者血清LRG1水平下降,GDF15和GP73水平升高,3者血清中的水平与原发性肝癌介入治疗后复发具有相关性,3者血清中水平联合检测对介入治疗后复发具有较高的预测效能。Objective To investigate the expression levels of leucine-rich-alpha-2-glycoprotein 1(LRG1),growth differentiation factor 15(GDF15),and Golgi protein 73(GP73)in the serum of patients with primary liver cancer and their predictive value for recurrence after interventional treatment.Methods A total of 221 patients with primary liver cancer undergoing interventional treatment from January,2019 to December,2022 were included as the observation group.These patients were separated into a recurrence group of 62 cases and a no-recurrence group of 159 cases based on whether there was a recurrence event.221 healthy volunteers who underwent physical examinations during the same period were included as the control group.Enzyme linked immunosorbent assay(ELISA)was applied to detect the expression levels of LRG1 and GDF15 in the serum of two groups,and the GP3 content was detected using the forward light method.Multivariate logistic regression analysis was applied to identify the influencing factors of recurrence after interventional therapy;Receiver operating characteristic(ROC)curves were drawn to evaluate the diagnostic value of serum LRG1,GDF15 and GP73 levels for primary liver cancer and the predictive value of recurrence after interventional therapy.Results Compared with the control group,the expression level of serum LRG1 in the observation group significantly decreased,while the expression levels of GDF15 and GP73 significantly increased(P<0.05).ROC curve showed that the area under the curve(AUC)of serum LRG1,GDF15 and GP73 levels in patients with primary liver cancer was 0.950,which was significantly higher than that of LRG1(Z=4.572,P<0.001),GDF15(Z=3.587,P<0.001)or GP73(Z=6.171,P<0.001)alone.Compared with the non-recurrence group,the serum LRG1 expression level in the recurrence group significantly decreased,while the expression levels of GDF15 and GP73 significantly increased(P<0.05).Multiple logistic regression analysis showed that extrahepatic metastasis,alpha fetoprotein,LRG1,GDF15,and GP73 were the influencing f
