摘　　要：目的评估血清基质金属蛋白酶-14(MMP-14)在结直肠癌(CRC)诊断中的价值。方法采用ELISA法检测81例CRC患者、30例结直肠腺瘤(CA)患者及81例健康对照者的血清MMP-14水平,分析CRC患者血清MMP-14水平与肿瘤临床病理特征的关系,并将其与癌胚抗原(CEA)进行比较。结果CRC患者血清MMP-14水平明显高于CA组和健康对照组(P<0.05)。CRC患者血清MMP-14水平随着肠壁浸润(T因子)、淋巴结转移(N因子)和远处转移(M因子)的存在而升高。血清MMP-14的ROC曲线下面积AUC(0.716)略低于CEA的AUC(0.755),血清MMP-14联合检测CEA的AUC最大(0.770)。结论血清MMP-14在CRC诊断中的作用与血清CEA类似,与CEA联合使用可提高诊断效能。Objective To assess the clinical value of the measurement of serum matrix metalloproteinase 14(MMP-14)in patients with colorectal cancer(CRC).Methods The serum levels of MMP-14 in 81 CRC patients,30 colorectal adenoma(CA)patients and 81 healthy controls were determined by ELISA method.We then evaluated their potential correlations with clinicopathological features of tumor,and also compared results with carcinoembryonic antigen(CEA).Results Serum levels of MMP-14 were significantly higher in CRC patients than in CA patients and healthy subjects.High serum level of MMP-14 was associated with increased levels of bowel wall infiltration(T factor),metastatic lymph nodes(N factor)and distant metastases(M factor).AUC value of MMP-14 was 0.716,slightly lower than AUC of CEA(0.755)in differentiation between CRC and healthy subjects.AUC(0.770)of the combination of MMP-14 with CEA was the highest.Conclusion Serum MMP-14,as a biomarker in the diagnosis of CRC,could have the similar value as CEA,especially in combination with CEA.
