摘　　要：目的探讨一例高分化子宫内膜样癌合并盆腔淋巴结淋巴管平滑肌瘤病及卵巢Brenner瘤的临床病理特点、诊断及鉴别诊断。方法回顾性分析2023年12月湖北省京山市人民医院收治的一例患者的临床表现、组织病理学特征及免疫组化表型,并复习相关文献。结果右侧卵巢肿瘤表现为弥漫分布的透明细胞巢,细胞核呈长椭圆形,核沟明显,核仁不明显。免疫组化示GATA-3、CK7、P63、P16阳性。双侧盆腔淋巴结内可见不规则、边界清晰的团块,内含透明类上皮细胞及梭形细胞交织,伴红染胶原束。免疫组化示ER、SMA、H-caldesmon、HMB-45阳性,裂隙内衬D2-40及CD34阳性类上皮样细胞。结论高分化子宫内膜样癌可合并盆腔淋巴结淋巴管平滑肌瘤病及卵巢Brenner瘤,诊断依赖临床表现、组织学形态及免疫组化检查。Objective To investigate the clinical and pathological features,diagnosis,and differential diagnosis of a case of well-differentiated endometrioid carcinoma combined with pelvic lymph node lymphangiomyoma and ovarian Brenner tumor.Methods A retrospective analysis was conducted on the clinical presentation,histopathological characteristics,and immunohistochemical profiles of a patient treated at Jingmen People’s Hospital,Hubei Province,in December 2023,along with a review of relevant literature.Results The right ovarian tumor showed diffuse distribution of transparent cell nests,with nuclei that were elongated oval-shaped,prominent nuclear grooves,and inconspicuous nucleoli.Immunohistochemical staining was positive for GATA-3,CK7,P63,and P16.Bilateral pelvic lymph nodes displayed irregular,well-defined masses containing intermingled transparent epithelial-like cells and spindle cells,with red-stained collagen bundles.Immunohistochemistry showed positivity for ER,SMA,H-Caldesmon,and HMB-45,and the irregular clefts were lined with D2-40 and CD34-positive epithelial-like cells.Conclusion Well-differentiated endometrioid carcinoma can coexist with pelvic lymph node lymphangiomyoma and ovarian Brenner tumor.Diagnosis relies on clinical presentation,histological morphology,and immunohistochemical examination.
