摘　　要：目的:了解在DRG运行背景下北京市某大型三甲公立医院2023年第2季度~2024年第2季度经济运营情况,分析内部结构及其变化,为公立医院进一步优化运营管理提供参考。方法:采用灰色关联和结构变动度分析,评价医院经济运营结构变化与各项收入之间的关系和密切程度。结果:医疗收入关联度排名前两位的是化验收入和医疗服务性收入,门诊和住院医疗收入关联度最高的分别为药品收入和化验收入。5个季度期间全院医疗收入结构变动度为2.90%,门诊收入结构变动度为4.71%,住院收入结构变动度为3.06%;医疗服务性收入对全院医疗收入波动贡献率最大。结论:在DRG支付模式下,公立医院应以公益性为核心,以经济运营结构调整为重点,合理管控药品和耗材,合理开展检查检验,探索完善临床路径,推动精细化管理落地实施,以进一步提升医院运营管理效能。Objective:To understand the economic operation of a large Grade III public hospital in Beijing from the second quarter of 2023 to the second quarter of 2024 under the DRG payment model,analyze its internal structure and changes,and provide references for further optimizing operational management in public hospitals.Methods:Grey relational analysis and structural variation degree analysis were used to evaluate the relationship and closeness between changes in hospital economic operation structure and various income components.Results:Among all revenue items,the highest correlations with medical income were laboratory income and medical service income.For outpatient and inpatient income,the highest correlations were with drug income and laboratory income,respectively.During the fivequarter period,the variation degree of total hospital medical income structure was 2.90%,outpatient income structure was 4.71%,and inpatient income structure was 3.06%.Medical service income contributed the most to fluctuations in overall medical income.Conclusion:Under the DRG payment model,public hospitals should focus on public welfare and prioritize adjustments to economic operation structure,including rational control of drug and consumables use,appropriate execution of tests and examinations,refinement of clinical pathways,and implementation of fine management practices to improve operational efficiency.
