摘 要:目的:通过数据包络分析评价在主诊医师负责制下手术科室的运行效率,为进一步发挥其积极作用提供理论依据。方法:选取C医院21个手术科室2023年运营数据和医疗资源配置数据作为投入产出指标,利用数据包络分析方法评估其综合效率、技术效率和配置效率。结果:C医院手术科室综合效率值为0.8571。8个手术科室(38.1%)规模报酬不变,12个手术科室(57.1%)规模报酬递增,1个手术科室(5.8%)规模报酬递减。结论:C医院以主诊医师负责制为主线,配套护理垂直管理、核算到医疗小组的绩效改革和床位调配制度,4大综合措施充分发挥了主诊医师负责制的积极作用,手术科室总体运行良好。为保障主诊医师负责制,未来还应及时调整医师配置使之适应业务规模。Objective:To evaluate the operational efficiency of surgical departments under the attending physician responsibility system using data envelopment analysis(DEA),and to provide theoretical support for further optimizing its effectiveness.Methods:Operational data and medical resource allocation data from 21 surgical departments of Hospital C in 2023 were selected as input and output indicators.DEA was used to assess their comprehensive efficiency,technical efficiency,and allocative efficiency.Results:The overall efficiency score of the surgical departments was 0.8571.Eight departments(38.1%)showed constant returns to scale,12 departments(57.1%)demonstrated increasing returns to scale,and one department(5.8%)had decreasing returns to scale.Conclusion:Hospital C has implemented a system centered on the attending physician responsibility model,supplemented by vertically integrated nursing management,performance reform down to the medical team,and bed allocation systems.These four comprehensive measures have significantly enhanced the system’s effectiveness,resulting in overall sound operation of the surgical departments.To sustain the model,future efforts should focus on timely adjustment of physician staffing to align with departmental workload.
