腹腔镜辅助远端胃癌根治术与全腹腔镜远端胃癌根治术的疗效比较

摘　　要：目的比较腹腔镜辅助与全腹腔镜远端胃癌根治术的治疗效果。方法选取2021年3月—2024年3月上海孟超肿瘤医院肿瘤外科收治的胃癌患者72例,依据手术方式的区别分为对照组(n=36)和观察组(n=36)。对照组采用腹腔镜辅助远端胃癌根治术,观察组采用全腹腔镜远端胃癌根治术,对比两组围手术期指标、近期疗效、炎性因子水平、远期疗效及并发症发生情况。结果各项围手术期指标在观察组均更优,差异均有统计学意义(P均<0.05)。近期疗效与远期疗效两组比较,差异均无统计学意义(P均>0.05)。观察组术后3 d各项炎症因子水平均低于对照组,差异均有统计学意义(P均<0.05)。并发症总发生率比较,观察组为0,对照组为16.67%(6/36),差异有统计学意义(P=0.025)。结论临床对胃癌患者采取腹腔镜辅助、全腹腔镜远端胃癌根治术治疗均能达到理想效果,但全腹腔镜远端胃癌根治术的炎症反应更少,并发症发生率更低。Objective To compare the therapeutic effect of laparoscopic assisted and total laparoscopic radical gastrectomy for distal gastric cancer.Methods A total of 72 patients with gastric cancer who were admitted to the Department of Surgical Oncology of Shanghai Mengchao Cancer Hospital from March 2021 to March 2024 were selected and divided into control group(n=36)and observation group(n=36)according to the difference in surgical methods.The control group was underwent with laparoscopic assisted radical gastrectomy for distal gastric cancer,and the observation group was underwent with total laparoscopic radical gastrectomy for distal gastric cancer.The perioperative indicators,short-term efficacy,inflammatory factor levels,long-term efficacy and complications were compared between the two groups.Results The perioperative indexes were better in the observation group,and the differences were statistically significant(all P<0.05).There was no significant difference in short-term and long-term efficacy between the two groups(all P>0.05).The levels of various inflammatory factors in the observation group were lower than those in the control group 3 days after surgery,and the differences were statistically significant(all P<0.05).The total incidence of complications was 0 in the observation group and 16.67%(6/36)in the control group,and the difference was statistically significant(P=0.025).Conclusion In patients with gastric cancer patients with laparoscopic assisted,total laparoscopic distal gastric cancer radical treatment can achieve ideal effect,but the laparoscopic distal gastric cancer radical less inflammatory response,lower incidence of complications.
