摘  要:目的探讨磁共振动态增强扫描联合弥散加权成像检查在诊断良恶性肌骨肿瘤中的价值。方法选取2021年9月—2023年9月北京积水潭医院贵州医院收治的94例疑似肌骨肿瘤患者为研究对象,均实施磁共振动态增强扫描、弥散加权成像检查。以病理结果作为金标准,对比分析磁共振动态增强扫描、弥散加权成像单项检查及两种方式联合诊断效能。结果经病理检查证实94例肌骨肿瘤患者中恶性肿瘤55例,良性肿瘤39例。联合诊断检出恶性肿瘤54例,良性肿瘤40例。磁共振动态增强扫描、弥散加权成像检查联合检测准确度为98.94%(93/94)、特异度为100.00%(39/39),均高于弥散加权成像单独检查的84.04%(79/94)、82.05%(32/39)和磁共振动态增强扫描单独检查的87.23%(82/94)、87.18%(34/39),差异均有统计学意义(χ^(2)=12.926、7.267,P均<0.05)。结论在临床中应用磁共振动态增强扫描、弥散加权成像检查诊断良恶性肌骨肿瘤均有各自优势,但与单项检测方式相比,两种方式联合的诊断效能明显更高。Objective To explore the value of magnetic resonance dynamic enhanced scan combined with diffusion weighted imaging in the diagnosis of benign and malignant musculoskeletal tumors.Methods A total of 94 patients with suspected musculoskeletal tumors admitted to Guizhou Hospital of Beijing Jishuitan Hospital from September 2021 to September 2023 were selected as the research objects.All patients underwent magnetic resonance dynamic enhanced scan and diffusion weighted imaging.The pathological results were used as the gold standard.The diagnostic efficacy of magnetic resonance dynamic enhanced scan,diffusion weighted imaging and the combination of the two methods were compared and analyzed.Results Pathological examination confirmed that there were 55 cases of malignant tumors and 39 cases of benign tumors in 94 patients with musculoskeletal tumors.54 cases of malignant tumors and 40 cases of benign tumors were detected by combined diagnosis.The accuracy and specificity of combined detection of dynamic contrast-enhanced magnetic resonance imaging and diffusion-weighted imaging were 98.94%(93/94)and 100.00%(39/39),which were higher than 84.04%(79/94)and 82.05%(32/39)of diffusion-weighted imaging alone and 87.23%(82/94)and 87.18%(34/39)of dynamic contrast-enhanced magnetic resonance imaging alone,and the differences were statistically significant(χ^(2)=12.926,7.267,both P<0.05).Conclusion Magnetic resonance dynamic enhanced scan and diffusion weighted imaging have their own advantages in the diagnosis of benign and malignant musculoskeletal tumors in clinical practice,but compared with the single detection method,the diagnostic efficiency of the combined detection of the two methods is significantly higher.
