经阴道超声和MRI 对子宫内膜癌分期的诊断效果研究

摘　　要：目的评估经阴道超声和MRI对子宫内膜癌分期的诊断效能。方法选取2022年10月—2023年10月成武县人民医院收治的98例疑似子宫内膜癌患者,均实行经阴道超声和MRI诊断,以病理检查结果为金标准,分析两种诊断方式的灵敏度、特异度、准确度及对子宫内膜癌不同分期的诊断符合率。结果病理检查结果确诊阳性80例、阴性18例;经阴道超声检出阳性71例、阴性27例;MRI检出阳性78例、阴性20例。MRI诊断的灵敏度为96.25%(77/80)、准确度为95.92%(94/98),均高于经阴道超声诊断的83.75%(67/80)、82.65%(81/98),差异均有统计学意义(χ^(2)=6.944、9.013,P均<0.05);两种诊断方式的特异度比较,差异无统计学意义(P>0.05)。MRI对不同分期的诊断符合率均高于经阴道超声,差异均有统计学意义(P均<0.05)。结论子宫内膜癌诊断中MRI的诊断效能更高,优于经阴道超声,且MRI对子宫内膜癌不同分期的诊断符合率更高。Objective To evaluate the diagnostic efficacy of transvaginal ultrasound and MRI in the staging of endometrial cancer.Methods Ninety-eight patients with suspected endometrial cancer admitted to Chengwu County People's Hospital from October 2022 to October 2023 were selected.All patients were diagnosed by transvaginal ultrasound and MRI.The pathological examination was used as the gold standard.The sensitivity,specificity,accuracy and diagnostic coincidence rate of different stages of endometrial cancer were analyzed.Results Pathological examination confirmed 80 positive cases and 18 negative cases.Transvaginal ultrasound detected positive in 71 cases and negative in 27 cases.MRI showed 78 positive cases and 20 negative cases.The diagnostic sensitivity and accuracy of MRI were 96.25%(77/80)and 95.92%(94/98),which were higher than 83.75%(67/80)and 82.65%(81/98)of transvaginal ultrasound,and the differences were statistically significant(χ^(2)=6.944,9.013,both P<0.05).There was no significant difference in the specificity between the two diagnosis methods(P>0.05).The diagnostic coincidence rate of MRI at different stages were higher than those of transvaginal ultrasound,and the differences were statistically significant(all P<0.05).Conclusion The diagnostic efficacy of MRI in the diagnosis of endometrial cancer is higher than that of transvaginal ultrasound,and the diagnostic coincidence rate of MRI in different stages of endometrial cancer is higher.
