摘 要:目的分析双源CT脑灌注成像在急性脑梗死诊断中的应用。方法选取2023年1—12月莒县人民医院收治的96例疑似急性脑梗死患者为研究对象,均进行双源CT脑灌注成像、磁共振弥散成像检查,以临床综合诊断结果为依据,比较两种检查方法的诊断结果和诊断效能。结果96例疑似急性脑梗死患者中,临床综合诊断结果检出阳性82例,阴性14例;双源CT脑灌注成像检查检出阳性83例,阴性13例;磁共振弥散成像检查检出阳性81例,阴性15例。双源CT脑灌注成像的灵敏度为98.78%(81/82)、特异度为85.71%(12/14)、准确度为96.88%(93/96),均高于磁共振弥散成像的87.80%(72/82)、35.71%(5/14)、80.21%(77/96),差异均有统计学意义(χ^(2)=7.893、7.337、13.142,P均<0.05)。结论双源CT脑灌注成像对急性脑梗死的诊断效能优于磁共振弥散成像,可为后续临床治疗方案的制订提供更精准参考依据。Objective To analyze the application of dual-source CT cerebral perfusion imaging in the diagnosis of patients with acute cerebral infarction.Methods A total of 96 patients with suspected acute cerebral infarction admitted to People's Hospital of Juxian from January to December 2023 were selected as the research objects.Dual-source CT cerebral perfusion imaging and magnetic resonance diffusion imaging were performed.Based on the results of clinical comprehensive diagnosis,the diagnostic results and diagnostic efficacy of the two methods were compared.Results Among 96 patients with suspected acute cerebral infarction,clinical comprehensive diagnosis results showed that 82 cases were positive and 14 cases were negative.Dual-source CT cerebral perfusion imaging detected 83 positive cases and 13 negative cases.Magnetic resonance diffusion imaging detected 81 positive cases and 15 negative cases.The sensitivity,specificity and accuracy of dual-source CT cerebral perfusion imaging were 98.78%(81/82),85.71%(12/14)and 96.88%(93/96),which were higher than 87.80%(72/82),35.71%(5/14)and 80.21%(77/96)of magnetic resonance diffusion imaging,and the differences were statistically significant(χ^(2)=7.893,7.337,13.142,all P<0.05).Conclusion The diagnostic efficacy of dual-source CT cerebral perfusion imaging for acute cerebral infarction is better than that of magnetic resonance diffusion imaging,which can provide a more accurate reference for the formulation of subsequent clinical treatment plans.
