非接触式广角镜在单纯裂孔性视网膜脱离患者玻璃体切除术中的应用

摘　　要：目的 探讨非接触式广角镜在单纯裂孔性视网膜脱离患者玻璃体切除手术中的应用效果及眼表的变化。方法 选取2020年1月—2022年12月云浮市人民医院眼科收治的拟行玻璃体切除术的单纯裂孔性视网膜脱离患者120例(120眼),根据手术方法不同划分成对照组和研究组,各60例(60眼)。对照组使用传统角膜接触镜,研究组使用Resight非接触式广角镜。比较两组手术效果、术后视力恢复情况、术后并发症发生情况、手术前后眼表变化情况。结果 研究组手术成功率为100.00%(60/60),高于对照组的85.00%(51/60),差异有统计学意义(P=0.003)。研究组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。研究组视力恢复良好率高于对照组,差异有统计学意义(P<0.05)。研究组术后泪膜破裂时间、角膜荧光素钠染色、泪液分泌试验均优于对照组,差异均有统计学意义(P均<0.05);两组患者角膜厚度对比,差异无统计学意义(P>0.05)。结论 相比传统角膜接触镜,手术中使用非接触式广角镜执行玻璃体切除,手术效果、患者术后恢复效果较好,可降低术后并发症风险,且对眼表影响较小。Objective To investigate the application effect of non-contact wide-angle lens in vitrectomy for patients with simple rhegmatogenous retinal detachment and the changes of ocular surface.Methods A total of 120 patients(120 eyes) with simple rhegmatogenous retinal detachment who underwent vitrectomy in the Department of Ophthalmology in Yunfu People's Hospital from January 2020 to December 2022 were selected.According to different surgical methods,they were divided into control group and study group,60 cases(60 eyes)in each group.The control group used traditional corneal contact lens,and the study group used Resight noncontact wide-angle lens.The operation effect,postoperative visual acuity recovery,postoperative complications and ocular surface changes before and after operation were compared between the two groups.Results The success rate of operation in the study group was 100.00%(60/60),which was higher than 85.00%(51/60) in the control group,and the difference was statistically significant(P=0.003).The incidence of postoperative complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).The rate of good visual recovery in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).The postoperative tear film rupture time,corneal fluorescein sodium staining and tear secretion test in the study group were better than those in the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in corneal thickness between the two groups(P>0.05).Conclusion Compared with traditional contact lenses,non-contact wide-angle lenses are used to perform vitrectomy during surgery.The surgical effect and postoperative recovery effect of patients are better,the risk of postoperative complications is reduced,and the ocular surface is less affected.
