半导体激光联合牙周基础治疗在重度牙周炎中的应用及其对牙周指标的影响

摘　　要：目的 分析重度牙周炎患者采取半导体激光联合牙周基础治疗的效果。方法 选取2022年11月—2024年5月安徽医科大学合肥口腔临床学院收治的重度牙周炎患者50例,根据不同的治疗方法分为对照组、观察组,各25例。对照组予以牙周基础治疗,观察组在对照组基础上使用半导体激光治疗。对比两组牙周指标、炎性因子、龈沟液骨代谢因子及安全性。结果 治疗后,观察组牙周探诊深度、牙周附着水平、牙龈指数、牙菌斑指数、超敏C反应蛋白、肿瘤坏死因子-α、白细胞介素-6水平均低于对照组,差异均有统计学意义(P均<0.05)。治疗后,观察组Ⅰ型前胶原氨基N端前肽水平为(47.18±4.16)ng/mL,高于对照组的(35.52±4.27)ng/mL,差异有统计学意义(t=9.780,P<0.05)。治疗后,观察组Ⅰ型胶原C端肽、降钙素原、骨钙素水平均优于对照组,差异均有统计学意义(P均<0.05)。两组不良反应总发生率对比,差异无统计学意义(P>0.05)。结论 重度牙周炎患者合用半导体激光、牙周基础治疗有利于改善牙周指标,降低炎性因子水平,安全性更为可靠。Objective The effect of semiconductor laser combined with periodontal basic treatment on patients with severe periodontitis were analyzed.Methods From November 2022 to May 2024,a total of 50 patients with severe periodontitis admitted to Hefei Clinical College of Stomatology,Anhui Medical University were selected and divided into control group and observation group according to different treatment methods,with 25 cases in each group.The control group was treated with periodontal basic treatment,and the observation group was treated with semiconductor laser on the basis of the control group.The periodontal indexes,inflammatory factors,gingival crevicular fluid bone metabolic factors and safety were compared between the two groups.Results After treatment,the periodontal probing depth,periodontal attachment level,gingival index,plaque index the levels of hypersensitive C-reactive protein,tumor necrosis factor-α and interleukin-6 of the observation group were lower than those of the control group,and the differences were statistically significant(all P<0.05).After treatment,the amino N-terminal propeptide of type Ⅰ procollagen level in the observation group was(47.18±4.16) ng/mL,which was higher than(35.52±4.27) ng/mL in the control group,and the difference was statistically significant(t=9.780,P<0.05).After treatment,the levels of type Ⅰ collagen C-terminal peptide,procalcitonin and osteocalcin in the observation group were better than those in the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of semiconductor laser and periodontal basic treatment in patients with severe periodontitis has significant advantages,which is conducive to improving periodontal indexes and reducing the level of inflammatory factors,and the safety is more reliable.
