老年食管癌患者术后下肢深静脉血栓发生情况及影响因素分析

摘　　要：目的探讨老年食管癌患者术后下肢深静脉血栓(DVT)发生情况,并分析其影响因素。方法选取85例老年食管癌患者,均接受食管癌根治术,记录术后下肢DVT发生情况。采用多因素Logistic回归模型分析老年食管癌患者术后发生下肢DVT的影响因素。结果85例老年食管癌根治术患者中,25例术后发生下肢DVT,发生率为29.41%(25/85),60例未发生下肢DVT。单因素分析结果显示,合并高血压情况、D-二聚体水平、术后卧床时间、淋巴结转移情况、频繁采血情况均可能是老年食管癌患者术后发生下肢DVT的影响因素(P﹤0.05)。多因素Logistic回归分析结果显示,合并高血压、术后卧床时间长、出现淋巴结转移、D-二聚体水平高、频繁采血均是老年食管癌患者术后发生下肢DVT的危险因素(P﹤0.05)。结论合并高血压、术后卧床时间长、出现淋巴结转移、D-二聚体水平高、频繁采血均是老年食管癌患者术后发生下肢DVT的危险因素。Objective To investigate the incidence of lower limb deep vein thrombosis(DVT)in elderly patients with esophageal cancer after surgery,and to analyze the influencing factors.Method A total of 85 elderly patients with esophageal cancer were selected,and underwent radical resection of esophageal cancer.The incidence of lower limb DVT after surgery were recorded.Multivariate Logistic regression model was used to analyze the influencing factors of lower limb DVT in elderly patients with esophageal cancer after surgery.Result Among 85 elderly patients with esophageal cancer undergoing radical esophagectomy,25 cases developed lower limb DVT after surgery,with incidence rate of 29.41%(25/85),while 60 did not develop lower limb DVT.Univariate analysis results showed that comorbidities of hypertension,D-dimer levels,postoperative bed rest time,lymph node metastasis,and frequent blood collection may be influencing factors for lower limb DVT in patients with elderly esophageal cancer after surgery(P<0.05).Multivariate Logistic regression analysis results showed that comorbidities of hypertension,long postoperative bed rest time,lymph node metastasis,high D-dimer levels,and frequent blood collection were risk factors for lower limb DVT in patients with elderly esophageal cancer after surgery(P<0.05).Conclusion The comorbidities of hypertension,long postoperative bed rest time,lymph node metastasis,high D-dimer levels,and frequent blood collection are risk factors for lower limb DVT in elderly patients with esophageal cancer after surgery.
