火龙罐综合灸在乳腺癌术后上肢淋巴水肿患者中的应用

摘　　要：目的探讨火龙罐综合灸在乳腺癌术后上肢淋巴水肿患者中的应用效果。方法根据干预方法的不同将86例乳腺癌术后上肢淋巴水肿患者分为常规组(n=43)和观察组(n=43),常规组患者采取综合消肿治疗(CDT),观察组患者采取CDT联合火龙罐综合灸治疗。比较两组患者的患肢臂围周径、上肢功能[臂、肩、手功能障碍(DASH)评分]及乳腺癌淋巴水肿相关症状[乳腺癌淋巴水肿症状指数量表(BCLE-SEI)]。结果干预后,观察组患者患肢臂围周径小于常规组,DASH评分低于常规组,差异均有统计学意义(P﹤0.05)。干预后,观察组患者症状发生个数和症状困扰情况均少于常规组,差异均有统计学意义(P﹤0.05)。两组患者症状困扰情况中,功能维度、社交维度、情绪维度比较,差异均有统计学意义(P﹤0.05),而睡眠维度和归属感维度比较,差异均无统计学意义(P﹥0.05)。结论火龙罐综合灸应用于乳腺癌术后上肢淋巴水肿患者中,可减小患肢臂围周径,改善上肢功能及乳腺癌淋巴水肿相关症状。Objective To investigate the application effect of fire dragon cupping comprehensive moxibustion in upper limb lymphedema patients after breast cancer surgery.Method According to different intervention methods,86 patients with upper limb lymphedema after breast cancer surgery were divided into conventional group(n=43)and observation group(n=43).Patients in the conventional group were treated with complex decongestion therapy(CDT),and patients in the observation group were treated with CDT combined with fire dragon cupping comprehensive moxibustion.The arm circumference of the affected limb,upper limb function[disabilities of the arm,shoulder,and hand(DASH)score]and the related symptoms of breast cancer lymphedema[breast cancer and lymphedema symptom experience index(BCLE-SEI)]were compared between the two groups.Result After intervention,the arm circumference of the affected limb and DASH score in observation group were lower than those in conventional group,and the differences were statistically significant(P<0.05).After intervention,the number of symptoms and symptom distress in the observation group were lower than those in the conventional group,and the differences were statistically significant(P<0.05).In symptom distress between the two groups,there were statistically significant differences in functional dimension,social dimension and emotional dimension(P<0.05),while there were no statistically significant differences in sleep dimension and belonging dimension(P>0.05).Conclusion In the treatment of upper limb lymphedema patients after breast cancer surgery,the fire dragon cupping comprehensive moxibustion can reduce the arm circumference of the affected limbs,improve the upper limb function and the related symptoms of breast cancer lymphedema.
